DUTIES AND RESPONSIBILITIES:


Under the supervision of the Program Director, the Peer Specialist provides support and mentoring, assisting participants by creating and following a plan for their continued recovery as they age. This role collaborates with other program staff and may assist in completing documentation, making service connections, escorting to appointments, and sharing resources that may help the participant maintain independence in their home.

Job responsibilities will include:


Provide support for clients of Living In Place

Conduct assessments, develop and maintain service plans, make referrals as deemed appropriate, and facilitate successful service connections.

Referral and cooperative relationships with other community agencies, to meet the needs of clients.

Ongoing reassessment of client need, monitoring of service efficacy, and appropriate revision of the service plan.

Escort clients to appointments

Informing supervisors of situations of crisis or circumstances that require further guidance.

Represents and interprets the agency at community conferences and meetings.

Participates in training and staff meetings.

Attend training as assigned in specific evidence-based treatments and as required by DOHMH and JASA.

Participate in training and unit meetings.

Completes required case records, reports, and statistics in compliance with JASA and DoHMH standards.

Performs such other duties as assigned.

QUALIFICATIONS:

High School Diploma or equivalent

Must be currently certified, or become certified, with either a Provisional or Professional Peer Specialist certification, within 12 months of hire

Good writing/record-keeping skills.

Ability to read, write, speak, and understand English. Ability to speak and understand Spanish is preferred.

Must be computer literate, including the ability to learn and use program-specific software.

Ability to travel to clients' homes and to negotiate any steps and barriers in clients' residences.

Understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients and families.
