SUMMARY: Works collaboratively with providers to improve the management of chronic disease via phone or in office as directed by the physician.

Required Education/Training: Associate's degree in Nursing
Required Experience: Two (2) years clinical experience
Required Licensure/Certification/Registration: Current Registered Nurse (RN) license with eligibility to work in Missouri; BLS

Preferred Education/Training: Bachelor of Science degree in Nursing, EHR; graduate from an accredited nursing program
Preferred Experience: Recent experience in a clinic or physician office setting, strong organizational skills, attentiveness to detail, highly motivated with the ability to multi-task
