Responsibilities

Works collaboratively with multidisciplinary team to assess patient's needs and barriers to care. Based on this assessment, actively works to develop and refer to appropriate external and internal resources. Evaluates and documents interventions provided to address needs and barriers, including follow up and utilization.

Qualifications

Required:

• One year as a Mastectomy Fitter
• Certified Mastectomy Fitter Must obtain Certified Mastectomy Fitter credential within one year of hire.

Desired:

• One year as a Certified Mastectomy Fitter.
• High School Diploma or GED
