Job Description
Overview
New Hope Services (NHS) is proudly celebrating 65 years of service to Southern Indiana and our extended service area. NHS has built the capacity to provide a full continuum of human services from child and family support through ability services. ESSENTIAL DUTIES:- Provide services that are essential to reducing the effects of a disability experienced by the consumer as identified on the Person Centered Individual Service Plan (PCISP) and as directed by supervisor or designee
- Assure Consumer dignity, confidentiality and safety within the program at all times
- Conduct classes, training and arrange opportunities for consumers according to PCISP goals and objectives.
- Maintain forms and tracking sheets for Park Place, including but not limited to: visitor sign-in logs, staff shift checklists, house drills, environmental checklists, client chore charts, client receipts and expense tracking sheets, food diaries and behavior tracking.
- Ensure home organization is maintained and inventory on needed items is communicated to management
- Attend all required meetings/staffing to include training, development, and in-services
- Operate program within all application regulations/laws and policies
- Assist in the daily self-care/personal care needs of specific program participants (i.e. toileting, feeding, lifting and transferring).
- Ensure that all individual billing and related documentation is submitted to management per program expectations. End of business day for on-site services, 36 hours from shift completion for off-site services.
- Assure that all services meet quality standards as outlined by your Supervisor
- Follow Transportation Policy when transporting consumers using either personal or company vehicles
- Communicate and work with other staff, in a professional manner, to coordinate program components and goals.
- Maintain daily communication with your supervisor
- Prepare program in conjunction with Manager and assure that plans are followed
- Must have access to the internet to maintain communication and submit needed documentation electronically
- Be accessible, flexible and cooperative to work in multiple direct care programs, as deemed necessary by your Supervisor
- Must have to ability to take the on call phone on nights and weekends in rotation with other department leadership.
GENERAL DUTIES:
- Assist when needed to ensure efficient operation of New Hope Services, Inc., Ability Services programs
- Responsible for familiarity and compliance with standards, policies and procedures as outlined by state, federal, CARF and New Hope Services
- Perform tasks as requested and held directly responsible to the Residential Services Manager.
- Serves as a positive role model, exhibits high ethical standards, and promotes the mission and values of NHS
- Maintain familiarity with and review the personnel policies and procedures of New Hope Services, Inc. (Employee Handbook)
- Contribute to the team effort to accomplish the mission of New Hope Services, Inc.
- Model appropriate language and behavior.
- Must possess a high level of patience and tolerance, and the desire to assist others to reach their potential
- Ability to work as a team member and take direction in order for team to accomplish goals and objectives
- Friendly, positive, cooperative, and helpful attitude
- Complete all annual tests and trainings on or before deadline, including but not limited to TB test or chest x-ray, SCM, First Aid/CPR, Core A/B & BBP Certifications, ISP/PCP, HCPS and Relias Modules
5. Must possess a valid state driver’s license, proof of insurance coverage per company policy, and have a vehicle that is reliable for transportation of consumers
- Experience with adults with developmental disabilities or functional impairments
EDUCATION/YEARS OF EXPERIENCE:
- High School Diploma or GED equivalent required
- 1 to 2 years of experience with individuals that have developmental disabilities and/or functional impairments required
