Position Summary

The CSNP Population Health Coordinator is responsible for supporting the identification, engagement, tracking, and ongoing management of Chronic Special Needs Plan (CSNP) eligible patients across the primary care network. This role serves as a key liaison between primary care clinics, agents/brokers, and operations leadership to ensure CSNP-eligible patients are appropriately supported throughout the enrollment and care journey.

This individual will also provide broad operational and project support to the Director of Operations as needed.

Key Responsibilities

CSNP Population Identification & Tracking

  • Maintain and manage a centralized tracking system for CSNP-eligible and enrolled patients.
  • Partner with clinics to identify eligible patients using EMR reports, payer lists, and internal dashboards.
  • Monitor patient movement through the CSNP pipeline from identification → outreach → enrollment → retention.
  • Track performance metrics and provide regular status updates and reporting.

Primary Care Coordination

  • Act as the operational bridge between primary care teams and CSNP initiatives.
  • Support clinics with workflows, documentation, and processes related to CSNP patient identification and engagement.
  • Help ensure patients are scheduled for required visits, screenings, and follow-up care.
  • Assist with closing care gaps and improving quality outcomes for the CSNP population.

Agent & Broker Collaboration

  • Serve as the primary operational contact for insurance agents and brokers working with CSNP patients.
  • Coordinate communication between brokers and clinic teams.
  • Track referrals and enrollment outcomes from broker partnerships.
  • Help ensure a smooth, compliant patient handoff between brokers and clinics.

Operational & Project Support

  • Provide administrative and project support to the Director of Operations.
  • Assist with workflow development, process improvement, and implementation of new initiatives.
  • Prepare reports, presentations, and data summaries.
  • Support cross-functional projects and special initiatives as assigned.

Key Performance Indicators (KPIs)

  • Growth and retention of CSNP patient population
  • Accuracy and timeliness of tracking and reporting
  • Clinic and broker satisfaction with coordination support
  • Completion of operational projects and initiatives
  • Improvement in care gap closure and patient engagement

Qualifications

  • Bachelor’s degree preferred (Healthcare Administration, Public Health, Business, or related field).
  • 2+ years experience in healthcare operations, care coordination, population health, or insurance.
  • Strong organizational and project coordination skills.
  • Comfortable working with data, reports, and tracking tools.
  • Excellent communication and relationship-building skills.
  • Experience with EMR systems and Medicare Advantage preferred.
