Job Description
Supervises assigned Day Services program area to ensure health, safety and appropriate habilitation efforts on behalf of individuals served in accordance with VALLEYLIFE Policy and Procedures.
Monday - Friday 7 am - 4 pm; I-17 & Greenway
Position Responsibilities
- Provides supervision and direction to program staff and ensures VALLEYLIFE Policy and Procedures are followed.
- Training of direct care staff, monitoring data on the effectiveness of the BTP program objective and may attend mediation reviews when necessary.
- Completes all required areas of the Individual Service Plan (ISP) and attends meeting. Ensure ISP's are followed.
- Maintains accurate member records and ensures all pertinent documentation is filed in each individual member's master file.
- Ensures that needs of individuals served are met by providing a safe, appropriately supplied and adequately staffed program.
- Ensures all staff is trained on the needs and communication of each member.
- Ensures timely orientation to DTA including the completion of the member site on the first day in the DTA.
- Ensures medication is administered per doctor's orders and medication is refilled as required to ensure adequate supply of medication. Residential group home notified a week in advance for refills.
- Follows all policies and procedures, standards and licensing per contract requirements.
- Follow all policies and procedures regarding attendance, including punctuality and notification.
- Participates in departmental meetings, inter-department meetings, committee meeting, and in-service training session as required.
- Assist with intake of new members & training staff on the new members.
- Maintains awareness of all rules and regulations and ensures compliance.
- Coordinates with supervisor information regarding members to parents, guardians, VALLEYLIFE and other agencies serving the member.
- Attend as necessary annual physical, medication reviews, wheelchair evaluation, and any other medical appointment as deemed necessary to meet the needs of the member.
- Performs additional duties as required, records attendance, billing and completes monthlies.
- Responsible for checking e-mail on a daily basis.
- Order supplies.
