We are seeking an experienced and compassionate Home Health Physical Therapist to join our dedicated team. The ideal candidate will have a passion for helping patients achieve their highest level of independence and mobility. This position allows you to work with autonomy, while also being supported by an interdisciplinary team focused on patient-centered care.

• Conduct comprehensive evaluations of patients’ physical and functional abilities

• Develop individualized care and treatment plans tailored to each patient’s goals

• Deliver expert therapy services in the patient’s home setting

• Document patient progress accurately and in compliance with regulations

• Collaborate with nurses, physicians, and other healthcare professionals to ensure continuity of care

• Educate patients and families on therapeutic techniques and safe practices for ongoing care

• Current licensure as a Physical Therapist (PT) in Virginia

• Minimum of 1 year of experience (home health preferred, but not required)

• Strong clinical and communication skills

• Ability to work independently and manage a flexible schedule

• Commitment to providing compassionate, high-quality care
