Position Overview We are seeking an experienced Home Health Physical Therapist to provide skilled therapy services to patients in their homes. This role offers autonomy, flexible scheduling, and strong interdisciplinary support while making a direct impact in the community.

Responsibilities • Conduct comprehensive PT evaluations • Develop and implement individualized treatment plans • Provide skilled therapy services in the home • Complete timely and accurate documentation • Collaborate with interdisciplinary care teams • Educate patients and caregivers on safety and exercises

Qualifications • Active Virginia Physical Therapist (PT) license • Minimum 1 year of PT experience (home health preferred) • Strong clinical and communication skills • Ability to work independently • Commitment to compassionate, high-quality care
