Job Description

Position Summary

Under general supervision, the Program Coordinator will assist with the planning, organizing, coordination, recruitment, implementation, and development of NKFM programs focused on the National Diabetes Prevention Program (DPP). . The Program Coordinator will (a) work closely with community organizations to identify and refer people to programs; and (b) coordinate and deliver DPP programs to improve health outcomes for people at risk of type 2 diabetes and those living with kidney disease or related conditions. They will maintain and develop partnerships and sponsorships in the community. The coordinator will also provide support to other evidence-based programs, as requested and will receive other assignments as directed.

Major Responsibilities:

Responsibilities of the Coordinator will include, but are not limited to, the following:
Program Planning and organization
● Organize public health education programs focused on nutrition and physical activity and chronic disease prevention and management
● Ensure adequate staffing for all health education activities
● Coordinate with partner agencies, including recreation facilities, senior centers, health care agencies, to plan and promote programs
● Plan, organize, develop and deliver on the slate of assigned NKFM programs in various community settings or distance learning (online)
● Monitor and evaluate program effectiveness and attendance
● Ensure programs are delivered with fidelity according to planned objectives and timelines
● Stay current with industry trends, best practices, and emerging technologies related to the program area.

Program Promotion and Outreach
● Promote health education programs to the community through various channels to increase participation
● Focus on participant engagement by actively recruiting for programs at community outreach events, meetings, and identifying opportunities to speak one-on-one to individuals about available programs.
● Respond to inquiries regarding NKFM programs by phone and email; ensure people are referred to the appropriate program; track inquiries in a database.
● Work closely with other NKFM Program Coordinators to enhance community outreach and engagement
● Collaborate on community development initiatives to support public health education

Recruitment and Training
● Recruit and supervise Special Program Associates for health education programs
● Provide ongoing support and oversight to ensure effective program implementation
● Serve as a supervisor for SPAs and other full-time/part-time NKFM staff who wish to develop knowledge of or lead programs.
● Develop and/or deliver training sessions or workshops to enhance the knowledge and skills of frontline staff in chronic disease management
● Assist in the creation and maintenance of training materials, guides, and resources

Administrative Activities
● Maintain accurate records of program activities and participant data
● Prepare and submit reports on program outcomes and progress
● Assist in the development of program budgets and manage expenditures within allocated budgets

Special Events & Other Responsibilities
● Assist in planning and organizing special events to promote community-based health programs
● Participate in and support special events as needed to ensure successful execution
● Always represent the NKFM appropriately in a professional manner

Project Support
● Coordinate with cross-functional teams within the Division and broader organization on departmental or organizational projects.
● Track project progress and report on key milestones and deliverables, as assigned.

Collaboration

● Collaborate with all DMP Managers and Senior Managers on programmatic issues related to fidelity, training, adaptations and program developers.
● Continually collaborate with Mission Collaboration Unit to ensure alignment with program messaging, outreach goals, and data reporting.
● Work closely with the Community Integrated Services division to address participant needs through the Resource Navigation department and, when necessary, collaborate with the Referral Management department to develop referral systems

Position Qualifications:
Education and Experience
● Applicable experience in public health or health promotion or a Bachelor’s Degree. Advanced degree or certifications in public health preferred. High School Diploma required.
● Basic knowledge in chronic disease management and its application to program delivery in community and clinical settings
● [1-2] years of developing and maintaining community and clinical partnerships (internal and external) and leading organizational projects
● Experience in a community-based, or public facing role preferred
● Ability to demonstrate proficiency in MS Office Suite computer programs
● Obtaining/Maintaining a Community Health Worker (CHW) certificate

● Basic knowledge of or willingness to learn disease management, prevention, or falls prevention, or similar programs

Working Conditions
● Office environment with occasional requirements to attend off-site meetings or events
● Regular partnership development work in the community
● Hybrid (remote, in person) optional.
● Ability to work under pressure and meet deadlines
● Some positions may require occasional travel

Physical Requirements
● Ability to sit for extended periods
● Ability to lift and carry up to 25 pounds
