Job Description
The RN Nurse Navigator serves as a clinical case manager and primary liaison between the outpatient hospital-based wound care center and the inpatient hospital setting. This role is responsible for ensuring seamless transitions of care for patients with acute and chronic wounds across the continuum, from inpatient admission through discharge and return to outpatient wound care services.
The Nurse Navigator collaborates closely with inpatient nursing staff, hospital case managers, discharge planners, physicians, and post-acute care providers to coordinate wound care plans, reduce gaps in care, prevent readmissions, and promote optimal healing outcomes. The position includes a combination of inpatient hospital rounding, outpatient wound care clinic support and staffing as needed, and community outreach and education in conjunction with the Program Director.
Qualifications Required- Current, unrestricted Registered Nurse (RN) license in the state of practice
- Minimum of 3–5 years of clinical nursing experience, preferably in wound care, case management, medical-surgical nursing, or care coordination
- Strong knowledge of wound assessment, treatment modalities, and chronic wound management
- Experience collaborating with interdisciplinary teams across care settings
- BLS and ACLS
- Wound care certification (e.g., WCC, CWCN, CWS) – Strongly preferred
- Case management or care coordination experience
- Experience in hospital-based in-patient and outpatient departments
- Excellent communication and interpersonal skills
- Strong organizational and time-management abilities
- Ability to work independently across multiple clinical environments
- Strong problem-solving and care coordination skills
