We specialize in delivering high acuity, expert level pediatric care directly in the home setting. Our nurses care for medically fragile children with chronic conditions and acute illnesses, helping families avoid prolonged hospital stays and experience the stability, dignity, and warmth of home life. We serve Monmouth, Ocean, Middlesex, Hudson, Bergen, Union, Somerset, Mercer, Passaic, Burlington, and Camden counties and hourly rates range from $36.00 - $45.00 depending on experience and credentials.

  • Deliver direct nursing care by monitoring patient conditions, recording vital signs, administering medications and treatments, carrying out physician orders, completing care interventions, and following established safety protocols and preventive measures.
  • Assess and document the child’s physical and emotional health status and report any changes to the nurse case manager or physician.
  • Collaborate with families and care teams on disease processes, medications, and care techniques as appropriate through the Case Manager.
  • Maintain accurate and timely clinical documentation in accordance with agency policies and state regulations.
  • Report clinical errors, adverse events, and safety incidents to the nurse case manager and agency in accordance with agency policy.
  • Follow infection control protocols and maintain a safe, clean, and supportive environment for the child.
  • Respond promptly to emergencies and implement appropriate interventions.
  • Arrive to assigned shifts on time and fulfill scheduled commitments; notify the agency of any inability to work a scheduled shift per agency policy.
  • Remain flexible to scheduling adjustments and adapt to changing patient and care needs as directed.
  • Maintain professional and respectful communication with agency staff, families, and the care team, and respond to agency communications in a timely manner.
  • Engage actively in clinical supervision, required check-ins, and agency-directed communications.
  • Comply with agency policies, applicable laws, and nursing standards of practice.
  • Participate in mandatory orientation, competency validation, and training sessions.
  • Perform other duties as requested.
Requirements:

Required:

  • Current and active RN license issued by the New Jersey Board of Nursing
  • Current, non-expired BLS/ CPR certification
  • Valid driver’s license and reliable transportation
  • Have the ability to use assessment and critical thinking skills
  • Completion of agency-provided (or agency-approved) training and competency assessment

Preferred:

  • Bachelor of Science in Nursing (BSN)
  • Pediatric experience with medically fragile or chronically ill children is a plus.

Work Environment:

  • Patient care is provided in private residences and any locations based on their medical needs, which may include but not limited to home, school, or doctor’s office.
  • Exposure to infectious agents, household allergens, or emotionally challenging environments may occur
  • Variable scheduling including evenings, weekends, and holidays may be required based on patient needs

Physical & Environmental Demands:

  • Extended, overnight, or rotating shifts depending on patient needs
  • Periods of low activity followed by sudden high demand situations
  • Maintaining alertness, judgment, and responsiveness throughout long shifts
  • Supporting transfers between bed, wheelchair, and other surfaces
  • Repositioning patients for comfort, safety, and clinical needs
  • Some patient handling tasks involve pushing, pulling, lifting or supporting weights of up to approximately 40 pounds as part of routine care
  • Extended periods of standing, walking, or moving within a home environment
  • Bending, reaching, kneeling, and adjusting equipment or patient positioning
  • Sustained manual dexterity for medication administration and equipment operation
  • Reach above shoulders/overhead (IV poles, monitors, etc.)
