This clinical leadership role combines direct cardiovascular patient care with a strong focus on clinical quality, education, and clinician development.
You will provide evidence-based cardiovascular care through a virtual care model while helping strengthen consistency and quality across the clinical team.
The position offers the opportunity to shape quality programs, clinical standards, onboarding experiences, and ongoing professional education.
You will conduct peer and chart reviews, provide constructive performance feedback, and serve as a trusted clinical resource for other practitioners.
Working alongside cardiologists, advanced practice providers, primary care clinicians, and cross-functional teams, you will help drive coordinated and patient-centered care.
The role also emphasizes continuous improvement, evidence-based practice, and the effective use of technology to improve cardiovascular outcomes.
This is an opportunity for an experienced Nurse Practitioner to combine hands-on clinical expertise with meaningful influence on care quality and clinician excellence.
- Conduct comprehensive cardiovascular patient assessments through telehealth, including medical history reviews, clinical evaluations, and appropriate diagnostic testing.
- Develop differential diagnoses and evidence-based treatment plans for patients with cardiovascular conditions, including hypertension, heart failure, arrhythmias, and coronary artery disease.
- Prescribe medications, order and interpret appropriate diagnostic tests, and adjust treatment plans based on patient progress and clinical findings.
- Manage chronic cardiovascular conditions remotely while monitoring outcomes and supporting patients in achieving long-term health goals.
- Educate patients and families about cardiovascular conditions, prevention strategies, medication management, nutrition, exercise, smoking cessation, and lifestyle modifications.
- Collaborate with cardiologists, primary care providers, APPs, nurses, and other healthcare professionals to ensure coordinated and comprehensive patient care.
- Lead and support clinical quality initiatives, including peer review, chart review, quality oversight, and performance improvement activities involving APPs and physicians.
- Develop, implement, evaluate, and continuously improve clinical quality programs, standards, and performance expectations.
- Conduct clinical chart reviews and use findings to identify improvement opportunities, promote adherence to evidence-based practices, and strengthen clinical consistency.
- Design and deliver clinician onboarding, training, continuing education, and professional development programs.
- Provide coaching, mentorship, and constructive performance feedback to clinicians while fostering accountability and clinical excellence.
- Serve as a trusted clinical resource for complex cases, evidence-based cardiovascular care, and clinical practice questions.
- Stay current with cardiovascular research, clinical guidelines, regulatory expectations, and emerging best practices, incorporating relevant developments into care and education.
- Participate in research, quality improvement, and other initiatives designed to improve clinical outcomes and patient experience.
- Maintain accurate, comprehensive, and compliant patient documentation, including assessments, treatment plans, progress notes, and other required records.
- Advocate for patient needs and preferences while promoting shared decision-making and accessible, compassionate communication.
- Support quality assurance programs and contribute to a culture of continuous clinical improvement.
- Master’s or Doctoral degree in Nursing from an accredited Nurse Practitioner program.
- Current, unrestricted Nurse Practitioner/APRN license and active national board certification, such as AANP or ANCC certification.
- Ability to obtain and maintain required state licensure.
- Strong clinical experience as a Nurse Practitioner, with demonstrated experience managing cardiovascular conditions such as hypertension, heart failure, arrhythmias, and coronary artery disease.
- Cardiology experience is strongly preferred.
- Demonstrated ability to independently assess patients, develop differential diagnoses, formulate evidence-based treatment plans, prescribe medications, and order and interpret diagnostic testing.
- Experience managing chronic cardiovascular conditions and monitoring treatment effectiveness over time.
- Experience with clinical quality initiatives, chart review, peer review, quality assurance, or performance improvement programs.
- Ability to translate quality findings into actionable clinical standards, education, coaching, and performance improvements.
- Experience developing or delivering clinician onboarding, training, continuing education, or professional development programs.
- Demonstrated ability to mentor clinicians, provide constructive feedback, and support professional performance and development.
- Strong understanding of evidence-based cardiovascular care and commitment to staying current with clinical research, guidelines, and emerging best practices.
- Experience delivering patient care through telehealth or another virtual care environment, with the ability to use technology effectively for assessment, education, and engagement.
- Excellent written and verbal communication skills, with the ability to collaborate effectively with cardiologists, PCPs, APPs, clinical leadership, and cross-functional teams.
- Strong patient advocacy skills and a commitment to shared decision-making and patient-centered care.
- Ability to communicate complex cardiovascular information clearly, compassionately, and appropriately for different patient needs.
- Strong leadership, mentoring, analytical, and problem-solving capabilities.
