Developmental Services Case Manager

Two Full-Time Contracts to August 2027 (with the possibility of extension or becoming permanent)
  • Status: Full-Time, Contract
  • Starting Hourly Rate: $32.13 / Hour
  • Office Hours: Monday - Friday - 8:30 am - 4:30 pm, 35 Hours per week
  • Location: Aurora, Ontario L4G 3S9
  • Office Expectations; This position requires:
    • A minimum of one in-office day per month to provide team coverage
    • Attendance at additional in-office meetings, training sessions, supervision meetings, and all-staff events as required
    • Applicants must reside within a reasonable daily commuting distance of the office and/or assigned service area and be able to attend the office as required for operational needs

Job Description


The Developmental Services Case Manager, as part of a team serving York Region is responsible for the providing intensive case management, advocacy, and support services to individuals aged six and older with developmental disabilities and/or dual diagnosis (developmental disability and mental health).

The primary function of this role is to support children, adults, and their families through a person-centered approach to service delivery, assisting clients in navigating the developmental services system, identifying and accessing appropriate community resources, and exploring available support options. Responsibilities include conducting needs assessments, developing and coordinating individualized service plans, facilitating referrals, and supporting skill development to enhance independence, community participation, and overall quality of life.


Position Requirements
  • Bachelor’s degree in social work or related discipline (i.e. Health Sciences)
  • Experience;
    • Minimum three years directly related front line experience
    • Experience working with individuals who have developmental disabilities is essential
    • Experience in assisting people to develop independent living skills and plan for more independent living is an asset
    • Crisis prevention and intervention experience
    • The ability to demonstrate sound clinical judgment
    • Familiarity with the local service system and its providers is required
    • Strong analytical, problem solving, conceptual, and facilitation skills
    • Excellent oral and written communication skills
    • Ability to use a variety of software tools, including Microsoft, Data Base, Internet and Email
    • Ability to maintain strict confidentiality of Agency information
  • Travel within the geographic area of York Region
  • Working conditions - Office work, public sites, service user home environments, and travel with and without service users
  • Second language is considered an asset
  • An understanding of cultural diversity is considered an asset
* The successful applicant will be required to provide a police clearance that meets agency standards
* A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
