LPN - MEDS CLINIC

Position Summary

The primary purposes of the Licensed Practice Nurse (LPN) are to provide direct patient care, patient and family teaching, and work cooperatively within the interdisciplinary health care team in maintaining standards of nursing care under the direction of the Registered Nurse (RN) and within the framework of the Ohio Nurse Practice Act.

Department Summary

The LPN provides care for the clinic patients in the outpatient setting at the system MEDS Clinics.

The Aultman Health System MEDS Clinics are ambulatory clinics that focus on improving patient care through education and chronic disease state management.

Qualifications

  • Graduate of an accredited school of nursing.
  • Current, active licensure as a Licensed Practical Nurse through the Ohio Board of Nursing.
  • Obtain and maintain current American Heart Association (AHA) training in Basic Life Support (BLS) utilizing Resuscitation Quality Improvement (RQI) is required.
  • IV Certification, preferred.

Responsibilities & Expectations

  • Demonstrates knowledge of and applies evidence-based practice.
  • Collects comprehensive data pertinent to the patient’s health or the situation.
  • Identifies and reports changes in patients’ status to Registered Nurse for evaluation.
  • Documents nursing activities accurately and in a timely manner.
  • Implements the identified plan, coordinates care delivery, and employs strategies to promote health and a safe environment under the direction of the Registered Nurse.
  • Administers treatments and other direct care; prepares and administers medications as prescribed; observes and evaluates patients’ responses to medications and treatments; and identifies and promptly communicates adverse reactions to the Registered Nurse.
  • Collects specimens as ordered.
  • Interacts with and contributes to the professional development of peers, colleagues, and students.
  • Collaborates with patient, family, and others in the conduct of nursing practice.
  • LPN is a patient advocate in conjunction with the other members of the healthcare team and fosters patient safety, comfort, and satisfaction.
  • Attains knowledge and competency that reflects current nursing practice and applicable policies.
  • Adhere to professional standards and guidelines, including ethical actions, relevant statutes, rules and regulations, during nursing practice.
  • Exhibits High Reliability Organization (HRO) tools and tones.
  • Responsible for timely review of emails, department communications and applicable learning assignments and completion as applicable.

Department Responsibilities & Expectations

  • Serve as a member of Aultman’s health care team that provides quality care to patients.
  • Maintain good working relationships with the clinic providers and other members of the health care team.
  • Greets, rooms and prepares patients for provider examination.
  • Cross trained to check-in/check-out patients, schedule appointments, work front desk and update insurance.
  • Obtains pertinent patient information including vital signs, medication review, medication dosing history and changes in current medical conditions.
  • Adheres to infection control policies and procedures and follow universal precautions.
  • Perform POC testing (INR, A1c, and blood glucose); maintains quality controls and necessary documentation.
  • Provides appropriate documentation in the patient's chart/electronic medical record (EMR)
  • Oversees appointment bookings for all clinic providers and ensures preferences are given to patients in emergency situations. Maintains timely flow of patients.
  • Assists with clinic appointment reminder and follow-up calls as needed/requested by providers.
  • Help process prior authorization of medications and testing and/or prepares paperwork for prescription assistance programs.
  • Responds to and triages incoming telephone calls.
  • Responsible for the preparation of exam rooms including re-stocking treatment areas. Identifies clinical and office supplies and equipment needed and orders when appropriate.
  • Reinforces/facilities patient education appropriate to the individual patient’s needs.
  • Tracks and records clinic metrics and dashboard data.
  • Notifies clinic provider immediately of out-of-range POCT results.
  • Performs other duties assigned by the Director of Pharmacy.
  • Participates in continuing education and professional development.
  • Attends and actively participates in staff meetings.
  • Maintains knowledge of, and complies with, nursing, medical and clerical policies and procedures.
  • Adheres to the standards and policies of Aultman Health System.
  • Maintains patient confidentiality.

Working Conditions

  • Meal and break times are coordinated with other staff members to maintain adequate coverage.
  • Shifts are scheduled or changed due to the staffing needs of the unit/department based upon volume and acuity.
  • Work weekends and holidays as assigned or as applicable based on department needs.
  • Cross training depending upon department needs.

Department Working Conditions

  • 7AM – 5PM Monday-Friday or hours as required by the job and community need. No holiday, weekend, or off-shift rotations.
  • Works collaboratively with clinic providers, referring physicians and other disciplines.
  • Subject to frequent interruptions and changes in priority of duties throughout the day.

Hazardous Exposure Category

Category I - Includes tasks that involve exposure to blood, body fluids, or tissues. All procedures or other job-related tasks that involve an inherent potential for spills or splashes of, or mucous membrane or skin contact with blood, body fluids or tissues are Category I tasks. Use of appropriate protective measures is required for every employee engaged in Category I tasks.
