Advanced Practice Provider (PA or NP) - General Rheumatology in Chicago, Illinois at One Health Partners PLLC

One Health is seeking a compassionate and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing rheumatology team. The ideal candidate will provide patient-centered care focused on the evaluation, diagnosis, and management of rheumatic, autoimmune, and musculoskeletal conditions.

Key Responsibilities

• Evaluate patients presenting with rheumatic, autoimmune, and musculoskeletal conditions.
• Obtain medical histories, perform physical examinations, and assess disease activity.
• Develop and manage individualized treatment plans in collaboration with supervising or collaborating physicians.
• Order, review, and interpret laboratory testing, imaging, and other diagnostic studies.
• Prescribe and monitor medications in accordance with applicable regulations and clinical guidelines.
• Monitor treatment effectiveness, medication safety, and disease progression.
• Educate patients on diagnoses, treatment options, medication management, and ongoing disease monitoring.
• Maintain accurate clinical documentation and timely follow-up on patient results and messages.
• Coordinate care with physicians, specialists, infusion services, and other healthcare professionals.
• Maintain compliance with applicable clinical, regulatory, and organizational requirements.

Required

• Current Illinois license, or eligibility to obtain prior to employment.
• Current DEA registration or eligibility to obtain.
• Current BLS certification.
• Strong clinical assessment, communication, and decision-making skills.
• Commitment to high-quality, patient-centered care

Preferred

• Experience in rheumatology or related experience.
• Familiarity with autoimmune and inflammatory conditions and immunomodulatory therapies.
• Experience with electronic medical records.

Compensation & Benefits

• Base salary: $100,000–$125,000 annually
