Clinical Integration Specialist – Palliative Care

Visiting Nurse Health System (Visiting Nurse) is seeking a Clinical Integration Specialist for our Palliative Care team who will serve as a strategic partner with the CEO, Executive Director, and Business Development leadership to promote and streamline internal referrals. The Clinical Integration Specialist (CIS) impacts the overall enterprise-wide clinical integration strategy, focusing on care coordination and collaboration, enhancing effective communication, and developing and implementing standardized protocols and guidelines. The CIS also fosters data sharing and analytics.

Impacts the reduction in Home Health hospitalization rates through proactive care coordination and referralsUtilize analytic data (Acclivity and Bamboo) and provide education to internal and external customers and stakeholders on the value of Hospice and Palliative Care.Serve on the Clinical Integration Committee and participate in the committee’s effort to advance the strategic goals.Increase internal Home Health referrals to Hospice and Palliative Care by 10% month-over-month.Increase cross-departmental utilization of Palliative Care by 10% quarterly, with a baseline of 2.43% overall across all clinical units.Meet the referral to admission conversion of 85%
