Responsibilities
The Care Coordination Specialist works under RN supervision to provide coordinated care services for injured workers with low to moderate complexity claims. This position ensures continuity of medical services and facilitates communication between all parties involved while supporting return-to-work efforts within workers' compensation guidelines.
KEY RESPONSIBILITIES
Care Coordination & Communication
Coordinate routine medical care needs for assigned injured workers under established protocols
Facilitate communication between injured workers, providers, employers, and adjusters for straightforward cases
Collect and document clinical information from healthcare providers and claimants
Schedule and coordinate medical appointments, diagnostics, and ancillary services
Communicate work status updates to injured workers, employers, and claims adjusters
Case Management Support
Follow established care plans and treatment protocols under RN supervision
Monitor injured worker adherence to treatment plans and compare progress to established guidelines
Maintain accurate documentation of medical history and case progress in WC system using approved formats
Facilitate return-to-work efforts through coordination with employers and providers
Escalate cases to RN supervision when clinical complexity increases beyond the scope
Administrative Functions
Maintain and update case review notes and documentation on all assigned cases
Complete time management tracking on all cases
Attend team meetings and training sessions as scheduled
Ensure cost containment through the utilization of in-network providers when appropriate
Support network provider coordination efforts

Qualifications
Education & Licensure:
High School Diploma required
Graduate of an accredited practical/vocational nursing program
Current compact LPN/LVN license in good standing
Experience:
2+ years of clinical experience required
Workers' compensation or disability management experience is highly preferred
Healthcare coordination and documentation experience preferred
Skills:
Demonstrated organizational, technological, and communication skills
Basic understanding of disability management principles
Computer proficiency, including Microsoft Office Suite (Word, Excel), keyboard skills, and data entry capabilities
PERFORMANCE EXPECTATIONS:
Manage assigned caseload of low to moderate complexity workers' compensation claims
Achieve established metrics for routine case resolution and return-to-work outcomes
Maintain accurate and timely documentation per department standards
Escalate cases appropriately when clinical complexity exceeds the scope of practice
Demonstrate professional growth through continuing education and training participation
Support team objectives and departmental goals
SUPERVISION & REPORTING
Works under the direct supervision of the RN Case Manager or the WC Nurse Supervisor. Receives guidance on complex cases and clinical decision-making. Expected to work independently on routine coordination tasks while maintaining regular communication with the supervising RN.
