Independent Nursing Care (INC) is seeking an ambitious, dependable, and compassionate Registered Nurse to join our leadership team as Director of Patient Services (DPS).

This is not a behind-the-desk nursing position. We are looking for someone who enjoys being involved, solving problems, supporting caregivers, working directly with clients and families, and collaborating with a team to provide exceptional care.

The right person will be hardworking, organized, responsive, and willing to take ownership. Home care doesn't always operate between 9 and 5, so this position requires someone who understands the importance of being accessible for occasional evening and weekend clinical calls and urgent situations.

As Director of Patient Services, you will provide clinical leadership and oversight for INC's Home & Concierge Care programs while working closely with the Director of Home & Concierge Care, Care Team Managers, nurse assessors, caregivers, administrative staff, and company leadership.

Your responsibilities will include:

• Oversee nursing services and help ensure safe, compassionate, high-quality client care.
• Review client assessments, reassessments, plans of care, case openings, discharge summaries, and physician orders.
• Conduct client assessments and home visits when needed.
• Provide clinical guidance and support to RNs, LPNs, Home Health Aides, Nurse Assessors, and Nurse Educators.
• Respond to clinical questions and help triage client, family, and caregiver concerns.
• Maintain communication with clients, families, physicians, healthcare providers, contracting organizations, and community partners.
• Monitor changes in client condition and ensure care plans and staff instructions are updated appropriately.
• Conduct supervisory visits and caregiver evaluations.
• Educate caregivers regarding client needs, clinical expectations, and HHA scope of practice.
• Review caregiver documentation and address charting or care deficiencies.
• Assist with interviewing, hiring, orientation, training, and development of nurses and Home Health Aides.
• Oversee and participate in INC's Home Health Aide Training Program, competency evaluations, skills reviews, and staff education.
• Assist with annual in-services and ongoing clinical education.
• Participate in Quality Improvement activities, chart audits, incident/accident follow-up, and complaint resolution.
• Help develop, review, and maintain clinical policies and procedures.
• Support compliance with NYS Licensed Home Care Services Agency (LHCSA) requirements and other applicable regulations.
• Participate in emergency preparedness and assist staff and clients during weather events, power outages, illnesses, and other emergencies.
• Provide occasional hands-on nursing services including wellness visits, wound care, blood draws, immunizations, and patient advocacy.

We are looking for more than someone who meets the minimum qualifications. We want an RN who takes initiative, follows through, communicates well, and wants to help build an exceptional home care program.
The ideal candidate is someone who:

• Is a Registered Professional Nurse licensed in New York State.
• Has approximately 5+ years of nursing experience, preferably including home care, case management, community health, or a related setting.
• Has 2+ years of leadership or supervisory experience preferred.
• Has hospital or other strong clinical nursing experience preferred.
• Is comfortable making clinical decisions and providing guidance to field staff.
• Understands—or is motivated to learn—NYS home care regulations and LHCSA requirements.
• Is highly organized and able to manage multiple priorities.
• Communicates professionally and compassionately with clients, families, employees, and healthcare professionals.
• Is comfortable with electronic documentation and learning technology.
• Is willing to make home visits and step in clinically when the team needs assistance.
• Understands that leadership in home care sometimes requires responding to evening or weekend calls and unexpected situations.
• Wants to work hard, grow professionally, and be an important part of a collaborative leadership team.
