Supervisory Report: Community Health Outreach Supervisor
Wage Classification: Non-Exempt
Job Summary:
A member of a team that provides mobile and site-based outreach and case management services to patients of La Red Health Center. Such services include but are not limited to: engagement and the provision of basic survival items, program enrollment including public assistance and the Community Healthcare Access Program (CHAP), crisis intervention, problem solving within the system of care, housing placement, treatment placement, participation in the development of individual services plans, transportation assistance, and the coordination and brokerage of services with community based and internal resources. This position is responsible for overseeing the documentation of all services and contacts provided to patients. This position may also participate in community forums that address relevant topics/issues regarding homelessness, mental illness, special needs housing, and other medical/social issues affecting our patient population.

- Informal counseling and family support.
- Provision of culturally appropriate health education using curricular guidelines and resources defined by the Sussex County Health Advisory Coalition.
- Advocacy for individual and community needs.
- Assuring people receive the service they need by providing and documenting the enabling services of translation, interpretation and transportation.
- Identifying community-based resources and other assistance programs and communicating those resources to individuals in the community.
- Record keeping and documentation on clients served, services rendered, and follow up needs.
- Cultural mediation between communities and health and human services.
Education and/or Experience:
- High School diploma or GED; associates degree in Human Services preferred
- Two (2) years previous experience in community based outreach services preferred.
- Strong knowledge of Sussex county communities and highways
- Delaware Drivers license with clean MVA driving record.
Language Skills:
- Bilingual skills preferred English/Spanish or English/French Creole
Performance Requirements:
Knowledge:
- Knowledge of local community.
- Understanding of standard use and care of office equipment and supplies.
- Knowledge of EMR system, medical record documentation requirements and state/federal laws related to the release of health care information.
Skills:
- Service coordination skills.
- Time management skills, motivated and ability to self-direct.
- Capacity building skills
- Translate for patients referred outside the La Red Center.
- Abilities:
- Ability to impart information and education to a wide variety of audiences; community members, health professionals, and service providers.
- Ability to connect to the community
- Ability to communicate effectively and develop rapport with patients and families to facilitate implementation of a care plan.
- Ability to develop materials, displays, posters etc. announcing outreach efforts and events.
- Ability to remain calm when emergencies occur.
- Ability to safely operate a motor vehicle with patient passengers.
Equipment Operated:
Company vehicles including 5 person vans and vehicles. Operate a wide range of standard office equipment. Computer use and proficiency required.
Work Environment:
Work environment varies from passenger /patient transport vehicle to businesses, schools and churches to Outreach office at Health center. Includes waiting rooms, exam rooms, class rooms, church halls and outdoor health and outreach efforts. Controlled lighting, air quality, and temperature settings when inside. Environmental hazards may include weather related exposures and unpredictable exposure to communicable diseases and biohazards.
Mental/Physical Requirements:
Involves standing, walking, kneeling, bending, grasping, manipulating and squatting and lifting (up to 50 pounds). Walking or standing may be more than four (4) hours per day. Must be able to work in a standard medical office setting including use of a computer, and standard office equipment. Must have ability to read computer screens, printed materials and hearing and speech to communicate in person as well as over the phone. Must be able to attend meetings at various sites when required. Stress can be triggered by workload or traffic.
