Wellness Nurse (LPN/RN) in Springfield, Massachusetts at Mason Wright Senior Living

Mason Wright Senior Living is looking for a caring and experienced 40 hours per week LPN or RN to work 12:30 pm to 9 pm to join our Health & Wellness team. If you enjoy working with older adults, supporting families, and being part of a team that makes a difference, we’d love to meet you!

What You’ll Do

• Monitor resident health, safety, and well-being
• Complete resident assessments, care plans, and documentation
• Oversee medication management and conduct weekly medication audits
• Train and support RCAs in the SAMM medication program
• Communicate with residents, families, physicians, and healthcare providers
• Respond to health concerns and emergency situations
• Support staff training, quality assurance, and regulatory compliance
• Help ensure residents receive safe, compassionate, and quality care

What You Bring

• Current Massachusetts LPN or RN license
• 1–2 years of experience with older adults and/or dementia
• At least 2 years of experience supervising CNAs, HHAs, RCAs, or similar staff
• Strong clinical, communication, and documentation skills
• Assisted living experience preferred

Schedule

• Monday–Friday | Every other weekend | Alternating holidays | Evening/overnight on-call rotation
• Flexibility to respond to resident needs outside of scheduled hours is required.

Pay Range: $35.00-$37.00/hour

In addition, we offer a $3.00/hour shift differential for the 1st shift on weekends and the 2nd shift weekdays (Mon-Fri) and $4.00/hour for weekends from 4 pm to 12:30 am on top of base pay.
