Job Description

Home Health Registered Nurse – Weekend Option $20,000 Sign on bonus

Department: At-Home Health Care
Schedule: Saturday, Sunday, and Monday — three 10-12 hour shifts
Reports To: Clinical Manager

Position Summary

The Home Health Registered Nurse provides skilled, compassionate nursing care to patients in their homes. This weekend-option position works Saturday, Sunday, and Monday for 10 - 12 hours each day. The RN completes comprehensive assessments, coordinates individualized plans of care, provides skilled nursing interventions, and supports safe transitions from the hospital to the home.

Essential Duties and Responsibilities

  • Complete Start of Care, Resumption of Care, Recertification, and Discharge assessments.
  • Perform comprehensive nursing assessments and complete OASIS documentation accurately and timely.
  • Provide skilled nursing services, including wound care, medication management, IV therapy, disease-process education, and other ordered treatments.
  • Review weekend referrals and communicate with hospitals regarding anticipated discharges.
  • Contact patients promptly to confirm discharge status, verify demographic information, review ordered services, and schedule visits.
  • Develop, implement, and update individualized plans of care in collaboration with physicians, patients, caregivers, and the interdisciplinary team.
  • Monitor patients for changes in condition and promptly communicate concerns to the physician and Clinical Manager.
  • Educate patients and caregivers regarding safety, medications, treatments, disease management, and emergency planning.
  • Coordinate care with nursing, therapy, social work, home health aides, physicians, and community resources.
  • Complete all clinical documentation according to agency policies, regulatory requirements, and established timeframes.
  • Obtain and follow physician orders and ensure services support the patient’s homebound status and skilled needs.
  • Assist with weekend admissions, urgent patient needs, and scheduling changes.
  • Maintain clear communication with the weekend on-call team to ensure all responsibilities are completed.
  • Participate in care conferences, quality-improvement activities, staff education, and competency requirements.
  • Follow infection-prevention, emergency-preparedness, patient-rights, privacy, and safety policies.
  • Perform other duties as assigned.

Qualifications

  • Graduate of an accredited registered nursing program.
  • Current Illinois Registered Nurse license in good standing.
  • Current CPR certification.
  • At least one year of nursing experience preferred.
  • Home health, medical-surgical, wound care, IV therapy, or community-based nursing experience preferred.
  • Knowledge of Medicare home health requirements and OASIS preferred.
  • Strong assessment, communication, organization, and time-management skills.
  • Ability to work independently and make sound clinical decisions in the home setting.
  • Proficiency with electronic medical records and mobile documentation.
  • Valid driver’s license, reliable transportation, and automobile insurance as required by agency policy.

Schedule and Holiday Expectations

  • Regular schedule of Saturday, Sunday, and Monday.
  • Three 10-12 hour shifts per week.
  • The employee is required to work designated holiday based on agency needs and the holiday staffing schedule.
  • Additional flexibility may be required based on patient needs, referrals, admissions, and agency operations.
  • Reliable attendance and consistent weekend availability are essential requirements of this position.

Physical Requirements

  • Ability to travel throughout the agency’s service area in varying weather conditions.
  • Ability to stand, walk, bend, reach, and safely assist with patient care.
  • Ability to lift, carry, push, or pull equipment and supplies consistent with agency requirements.
  • Ability to perform duties in a variety of home environments.
