Job Description
The Pharmacy Care Coordinator reports to the Director of Pharmacy Programs and supports pharmacy-related Medicare Star Ratings measures and Healthcare Effectiveness Data and Information Set (HEDIS) quality performance initiatives by performing member outreach to identify, address and close care gaps. This role serves as a liaison between providers, patients, pharmacies, health insurance payers, and internal teams to ensure timely access to prescribed medications while supporting pharmacy operations in care coordination, provider assistance and program execution. Supports medication adherence and coordinates refills and patient education while maintaining compliance with healthcare regulations.
SPECIFIC SKILLS NEEDED
Proficient level of computer, verbal/written communication, and ability to prioritize
Effectively read, write, and speak English fluently
Attention to detail and strong organizational skills
Ability to manage high-volume workflows
Ability to work collaboratively with interdisciplinary teams
Ability to prioritize tasks and meet turnaround time expectations
Proficiency with EHR systems and pharmacy software
Excellent customer service and patient advocacy skills
Knowledge of medical and pharmaceutical terminology
EDUCATION/EXPERIENCE/TRAINING
Required:
High school graduate or equivalent
Minimum of 1 year of working experience in a healthcare related setting
Preferred:
Experience using Microsoft applications
Pharmacy-related experience
Experience in managed care or healthcare coordination
Knowledge of quality measures (Medicare Star Ratings and HEDIS) related to medication use
