Day Center Coordinator LPN - Full Time
Position Summary:
Under the supervision of the Center Administrator, the Day Center Coordinator LPN, in conjunction with the Health Care Coordinator (RN) or Licensed Nurse, is responsible for supervising the Personal Care Attendants (PCAs) and CNA's in the provision of activities of daily living, day center activities, restorative and supportive care of the EveryAge PACE participants. Monitors and provides a safe environment for participants. Looks for ways to improve the quality and effectiveness of systems and processes at the center.
Key Job Functions:
- Recruit, schedule, and retain qualified Personal Care Assistants ()/CNA to meet participant care needs and staffing goals.
- Assist with PCA/CNA training, competency validation, performance evaluations, and coaching in partnership with the Licensed Nurse.
- Oversee daily day center operations, including staffing coverage, participant activities, meal service, and a safe, clean environment.
- Ensure compliance with CMS, state, federal, and organizational regulations, policies, and quality standards.
- Monitor quality assurance and performance improvement initiatives, complete audits, and support corrective action plans.
- Address employee, participant, and family concerns while promoting excellent customer service and participant satisfaction.
- Collaborate with the interdisciplinary team (IDT) to communicate participant needs and support quality care delivery.
- Support workplace safety programs, incident reporting, and risk management activities.
- Participate in administrative on-call rotation and perform other duties as assigned.
QUALIFICATIONS:

- Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
- Must have a valid driver’s license, proof of insurance and have means of transportation.
Education:
- Bachelor's degree preferred. High school diploma required.
- Cardiopulmonary Resuscitation (CPR) certification and First Aid Certification required.
- Graduate of a school of professional nursing.
- Current North Carolina Licensed Practical Nurse (LPN) License.
Experience:
- A minimum of one year's experience working with the frail or elderly population.
- One year of management experience, preferably in a geriatric care setting required.
