Description:

The Elizabeth Hospice Nurse Practitioner provides compassionate and experienced palliative care to patients with serious and life-limiting illnesses. Consult with the palliative care medical director, to assesses, and develops a plan of care that align with the patient’s overall health status, goals and values, while collaborating with the IDT (interdisciplinary team), patient, family, and community physicians . Responsible for carrying out the mission, vision, values, and commitment to quality of The Elizabeth Hospice.

The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. We serve all of San Diego county and south Riverside county with multiple hospice office locations.

Our Mission: To enhance the quality of life for those nearing the end of life's journey and for those who grieve.

Salary: $100,000-$130,000; Subject to experience and qualifications.

Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m.

On Call Requirement: Two on-call shifts required per month.

Work Location: San Diego County, CA

Travel: Required for patient visits.

Requirements:

Responsibilities:

1. Responsible for conducting comprehensive palliative care assessments in home.

2. Gathers history and physical exam and diagnostics as needed, and then develops and implements plans of care given the patient’s goals of care and current conditions.

3. Develop, implement, and evaluate individualized care plans in collaboration with patients, families, primary care providers, specialists, and interdisciplinary team members.

4. Provide education and counseling to patients, caregivers, and families regarding disease progression, medication usage, symptom management, treatment options, side effects, and community resources.

5. Facilitate advance care planning discussions, including goals-of-care conversations, advance directives, and POLST documentation.

6. Ensure documentation is entered into the electronic record within expected guidelines.

7. Coordinates care with the interdisciplinary Team (IDT).

8. Participate in interdisciplinary team meetings, case conferences, quality improvement initiatives, and value-based care performance reviews.

9. Implements and maintains established policies and procedures as well as Standards of Care and regulatory requirements.

10. Support population health initiatives focused on improving quality metrics, patient engagement, and care outcomes.

11. Serve as a clinical resource and educator for staff regarding palliative care principles and best practices.

12. Maintains contact with IDT, patients’ other physicians and patients’ other medical providers to coordinate optimal care and resources for the patient and his or her family in a timely basis according to health plan coverage.

13. Orients, instructs, and trains assigned personnel.

15. When necessary, share in providing 24 hours, seven days a week coverage to patients and families.

16. Available rotating weekends to make patient visits as needed.

17. Attends medical staff meetings, nurse practitioner meetings and other meetings as directed by the CMO and Medical Services Director.

18. Consults the Palliative Care Medical Director when a plan of care is outside of standardized practice and protocols, and specialty consults are needed to manage patients care.

19. Counsels patients, families, and/or caregivers as to preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team to meet the total needs of patients.

20. Documents comprehensive medical history in patient’s electronic record. Identify pertinent patient information, progress, medications, and diagnostic studies as required. Keeps the interdisciplinary team updated on changes made to patients plan of care (per agency documentation standards).

21. Establishes, builds, and nurtures relationships with staff and community referral sources to facilitate program growth. Participates in community events as requested by the marketing team.

22. Adhering to the E-way standards is a requirement in performing all job duties

Qualifications:

1. Must be licensed as a Registered Nurse by the State of California.

2. Must be licensed as a Nurse Practitioner by the State of California.

3. Must have Drug Enforcement Administration (DEA) Certificate.

4. Must have valid National Provider Identification (NPI) Number or must be obtained within two (2) months of hire date.

5. Must have successfully completed a Nurse Practitioner (NP) Program from a recognized university and obtained a Master of Science Degree in Nursing (MSN). (Starting in year 2000, NP Program is included in the MSN).

6. National Certification required or must be obtained within two (2) months of hire date: American Association Nurse Practitioner (AANP) or American Nurses Credentialing Center (ANCC).

7. CPR/BLS Card.

8. Advanced Certification as Hospice and Palliative Care Nurse (ACHPN) preferred but required within one year of hire date.

9. Requires a minimum of three (3) years nursing experience in a hospice or palliative care setting.

10. Computer skills (word processing) use of adding machine, calculator, fax, photocopier.

11. Bi-lingual (Spanish) preferred.

12. Current California driver’s license and proof of insurance required.
