Job Description

Crest Nursing Home is seeking an Admissions Coordinator and Social Worker.
This Management position is part of our Interdisciplinary Team. Degree not required, but preferred, or experience in case management.

Primary and Essential Responsibilities:

Receives all inquiries for admissions to the facility. Collects data from preadmission interviews and hospital visits for the purpose of assessing the medical and psycho-social factors associated with the residents' condition. Completes admission paperwork. Coordinates admission process including communicating pertinent information to appropriate departments, arranging transportation, and assisting in obtaining any special needs. Develops and maintains the daily census. Interviews residents and/or their families to complete a psycho-social history and assessment of the resident, identifying problem areas and developing treatment plans and goals. Provides financial counseling to resident and/or their families regarding eligibility for financial assistance programs available to the residents.

Assists residents/family in dealing with anxieties and stress caused by illness and admission to the facility, emotional problems, difficulties in coping with residential physical disabilities, fear related to helplessness, death, and the need for institutional and specialized care. Performs documentation duties as needed. Schedules family conferences with resident, family and/or legal representative, and staff as needed. Participates in discharge and the development and implementation of a comprehensive discharge. Assists residents and families in utilizing community resources which helps them resume life in the community or to learn to live with their condition.

Maintains a good working rapport with all facility personnel and outside community health, welfare, and social agencies to assure that Social Services programs can be properly maintained to meet the needs of the residents. Relays the social, psychological and emotional needs of the residents and/or their families to the health care staff. Serves on various facility committees as required by current regulations, and as may be appointed by the Administrator. Serves as a public relations awareness representative for the facility.

Maintains confidentiality of all pertinent resident care information to assure residents' rights are protected. Actively participates in survey process by instructing staff in matters of conduct and disclosure, maintaining a presence at all times surveyors are on-site and directing the timely collection of information required by the survey team. Undertakes corrective action while survey is in progress if appropriate. Gathers and presents supplemental documentation to avoid potential deficiencies. Collaborates with Administrator to develop responses to survey report as needed. Participates in the development of the department budget. Provides relevant financial information to Administrator regarding department financial needs and status. Attends continuing professional education programs and in-service education. Applies information to current job responsibilities. Complies with all federal, state, local and Facility regulations and procedures.

Required Skills and Abilities:

Ability to communicate well both verbally and in writing.

Past experience in a health care setting preferred.

Must be free from convictions of abusing, neglecting, or mistreating individuals in a health care related setting.
