About the Role

The LPN or CMA Care Navigator is embedded on-site in the clinical office, working shoulder-to-shoulder with the pharmacist to support medication management, patient education, and day-to-day care coordination.

This is a hands-on clinical support role for an LPN or CMA who wants to build lasting patient relationships and be part of a small, tightly coordinated care team.

What You'll Do

• Support the clinical pharmacist with medication reconciliation, adherence checks, and care plan follow-through

• Conduct in-person and telephonic patient check-ins as part of the CCM/RPM program

• Take and document vitals, and monitor patient data from remote monitoring devices

• Educate patients on their conditions, medications, and self-management tools

• Coordinate with physicians and outside providers to keep care plans current

• Maintain accurate, timely documentation in the EHR and care management platform

• Flag clinical concerns to the pharmacist or supervising provider promptly

Required Qualifications

• Active LPN license in North Carolina

• Active Medical Assistant Certification in North Carolina

• Comfortable working in a fast-paced outpatient setting

• Strong communication skills and attention to detail

• Working proficiency with EHR systems and digital health platforms

Preferred Qualifications

• Prior experience with Chronic Care Management or Remote Patient Monitoring programs

• Experience in a clinic, care management, or ambulatory pharmacy setting
