In this fast-paced office role, you'll serve as the primary liaison between hospitals, physicians, referral sources, patients, families, and our clinical teams to ensure a smooth and timely admission process. If you're organized, detail-oriented, and enjoy coordinating care, we'd love to meet you!

• Receive and process new patient referrals accurately and efficiently.
• Verify insurance benefits, payer sources, and obtain required authorizations.
• Enter and maintain patient information within the Electronic Medical Record (EMR).
• Coordinate admissions with Nursing Supervisors and interdisciplinary clinical teams.
• Communicate professionally with referral sources, physicians, patients, families, and staff.
• Ensure compliance with agency policies, Medicare guidelines, and documentation standards.
• Participate in quality improvement (QAPI) initiatives and ongoing education.

• Current Texas LVN/LPN License
• Minimum one year of referral intake experience preferred
• Experience with Medicare, Medicaid, and third-party insurance verification preferred
• Medical office or healthcare background with knowledge of medical terminology
• Strong computer, organizational, and customer service skills
• HomeCare HomeBase experience preferred
• Valid Driver's License and Auto Insurance
