Job Description
Nurse Partner
The nurse partner performs comprehensive nursing assessments and provides nursing care in the clinic, community or home setting. Proactively monitors the health status and needs of a panel of patients over time and across the continuum of care. Develops, implements and modifies individual comprehensive plans of care in collaboration with the patient, primary and specialty care physicians and other providers. Authorizes services and refers to the appropriate medical, health and community providers while coordinating provider efforts and payment sources. Provides patient and family education. Synthesizes clinical information and utilization data from a variety of sources toward ensuring the provision of and/or transition to the appropriate level of care. A bachelor''s degree in nursing and 3 years in medical surgical or home health nursing -or an associate''s degree/diploma in nursing and 5 years in medical surgical or home health nursing is required, as is licensure in the state of Illinois as a registered nurse.
