QUALIFICATIONS
- Must have a Master’s Degree in Social Work or Counseling
- Must hold a valid license in the State of Ohio as an independently licensed Social Worker (LISW) or Professional Clinical Counselor (LPCC)
- Must meet all credentialing and privileging requirements as mandated by policy.
- Must maintain appropriate continuing education requirements.
- A minimum of two years of experience in a mental health treatment setting is required; experience in which behavioral health is integrated into primary care is preferred.
- Experience in the assessment and treatment of both mental health and chemical dependency is preferred.
- Must have proficient computer skills and the ability to become proficient in software programs specific to Lorain County Health & Dentistry i.e. NextGen, EPM, EHR, etc.
- Must have valid driver’s license and satisfactory driving record.
- Bilingual Spanish speaking is a plus
ESSENTIAL FUNCTIONS
- Evaluate and assess the mental health and substance use related symptoms of medical patients.
- Intervene in crisis situations during business hours.
- Provide brief assessments to clarify behavioral health diagnoses and to inform care plans.
- Define targeted treatment goals with and for the patients
- Provide short-term counseling to an established schedule of Lorain County Health & Dentistry patients, which may also include family members.
- Identify patients with more intensive behavioral health needs and assist these patient in connecting to appropriate care within the community.
- Ensure all plans and strategies developed are personalized, age specific, and culturally appropriate.
- Provide education, counseling, and support to patients using trauma informed and research based interventions whenever possible.
- Develop positive, professional relationships with patients.
- Promote good health throughout the facility in conversation and behavior.
- Accurately and concisely document all patient contact on a timely basis in medical record.
- Collaborate with medical and psychiatric providers to ensure integrated care.
- Participate in a Peer Review Process to ensure quality and consistency of BH service delivery
- Comply with HIPAA and 42 CFR guidelines as all times.
PHYSICAL DEMANDS
- Some lifting of supplies and equipment up to 50 lbs.
- 25% walking or standing, 75% sitting at a desk.
- Category II re: exposure to blood, bodily fluids, and communicable diseases.
PUBLIC AND PROFESSIONAL ACTIVITES
- Local travel required.
- Attend relevant professional activities such as company sponsored events as needed.
- Attend trainings and seminars as directed.
- Attend meetings as directed.
