Licensed Practical Nurse - Residential in Cortland, New York at The Arc Madison Cortland

The Licensed Practical Nurse (LPN) is responsible for the provision of basic nursing services, i.e medication administration, first aid health observation, and reporting of health and safety concerns relating to program individuals.

Responsibilities

• Coordinate and oversee the general health and medical management of assigned individuals, ensuring appropriate follow-up on medical issues and communication with physicians, clinicians, family members, and residential staff.
• Provide nursing support to residential programs by coordinating medical appointments, managing healthcare documentation, tracking follow-up care, and maintaining accurate medical records.
• Monitor and track medical appointments, laboratory testing, and clinical outcomes, partnering with the Registered Nurse to ensure timely follow-up and continuity of care.
• Assist in implementing and monitoring Nursing Plans, Nutrition Plans, and other health-related interventions for individuals receiving services, under the direction of the Registered Nurse.
• Support medication management activities, including medication orders, reconciliation, documentation, and reporting discrepancies to the Registered Nurse.
• Administer medications and provide nursing interventions as needed in response to special circumstances, emergencies, or program requirements within the scope of LPN practice.
• Maintain regulatory compliance, incident reporting, confidentiality, and adherence to OPWDD and agency standards.

• Licensed and currently registered as a Licensed Practical Nurse by the New York State Department of Education. Experience working with intellectually/developmentally disabled is preferred.
• A valid driver's license and good driving record acceptable to the Agency's insurance carrier.
