Salary: $43,920 – $45,355 annually, depending on qualifications and experience

About the Role
Burke is seeking a compassionate and flexible Wrap Care Coordinator to join the team at the Polk Mental Health Clinic in Livingston, TX. This position works closely with clinicians, families, community partners, and support systems to help children and adolescents remain safely in their homes and communities.
The Wrap Care Coordinator provides case management, assessments, team building, crisis support, and problem-solving services in accordance with individualized Wrap treatment plans. This role focuses on supporting youth who are at risk of out-of-home placement by strengthening family supports and coordinating community-based services.
The position requires strong collaboration with families, the Department of Family and Protective Services, Juvenile Justice, schools, and other community agencies to ensure children and families receive the support they need.
What You’ll Do
• Coordinate Wraparound services for children and adolescents with behavioral health needs
• Conduct assessments and assist with treatment planning
• Provide case management and crisis intervention services
• Build collaborative teams with families and community providers
• Help families identify strengths and develop problem-solving skills
• Coordinate services with DFPS, Juvenile Justice, schools, and community agencies
• Conduct emergency assessments as needed
• Maintain timely and accurate documentation in accordance with Medicaid and agency standards
• Provide support and advocacy to families and consumers
What We’re Looking For
• Bachelor’s degree in psychology, social work, medicine, registered nurse, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, early childhood intervention, or other Human Services discipline that meets QMHP requirements, from an accredited college or university
• Strong communication and teamwork skills
• Ability to work collaboratively with families and community partners
• Flexibility, creativity, and strong organizational skills
• Ability to maintain timely and accurate documentation
• Valid Texas driver’s license
What Helps You Stand Out
• Six (6) months of direct care experience working with children or adolescents in a behavioral healthcare or educational setting
• Knowledge of child and adolescent growth and development
• Familiarity with DSM-IV diagnostic criteria
• Licensed Bachelor Social Worker (LBSW) credential

Work Schedule
• Monday–Friday
• 8:00 AM – 5:00 PM
• Occasional after-hours work may be required to meet job responsibilities
Licenses & Certifications
• Valid Texas driver’s license required
• Must be physically able to complete SAMA, CPR, First Aid, and Defensive Driving training
• Driving record must be insurable by Burke’s insurance administrator
• Reliable personal vehicle required with routine travel throughout the service area
Physical Requirements
• Ability to lift up to 50 pounds
• Prolonged standing, walking, and driving
• Adequate sight, hearing, speech, and hand/finger dexterity
