About This Position

Job Summary:

  • Obtains medication histories for new patients and documents them in the electronic medical record in a timely manner.
  • Medication history will be obtained from patient, patient’s family, pharmacy, physician office, or long term care facility. Employee must be diligent to fully explore the medication history.
  • Communicates with pharmacist appropriately including asking questions, and referring for additional interview or intervention.
  • Uses good clinical judgement to update the medication history to the best possible list.
  • Demonstrates knowledge and skills to appropriately communicate and interact with employees, patients, families and visitors of all age groups while being sensitive to their cultural and religious beliefs.
  • Included within this role assignment is the responsibility to be a positive role model for all peers always reflecting the WE CARE philosophy.

  • Demonstrates effective communication skills and interaction with staff.

  • Demonstrates initiative to maintain/meet licensure and certification requirements.
  • Demonstrates ability to perform medication history as deemed appropriate by management and supervising pharmacist.

Qualifications:

Education/Skills:

Licensed Practical Nurse

Pharmacy Technician.

Work Experience:

Two years Retail pharmacy experience required.

One year LPN experience required.

Certification/Licensure-DUE UPON HIRE

  • Registration with the Mississippi State Board of Pharmacy.
  • Licensed with Mississippi State Board of Nursing

Mental Demands:

Ability to perform repetitive tasks, cooperate with others, take orders and follow directions

precisely, and demonstrate initiative. Ability to perform work accurately with attention to

detail within specified time periods. Ability to handle frequent interruptions and adapt to

changes in workload. Ability to set priorities and solve problems. Ability to respond quickly

to emergency requests. Ability to assist in meeting the pharmaceutical care needs of neonatal

pediatric, adolescent, adult and geriatric patients. Ability to perform in the conditions of an emergency

department.
