As a Primary Care Provider at PACE North, you will:
- Work as part of a interdisciplinary team and collaborate with other professionals to meet the emotional, physical, social, and medical needs of participants
- Deliver care in multiple settings:
- On-site clinic
- Participant homes
- Partner skilled nursing facilities
- Build strong relationships with participants and their families through education and support
- Practice in a flexible care model that adapts to participant needs rather than rigid systems

- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current and valid Michigan license as a Nurse Practitioner or Physician Assistant

- A collaborative mindset with a focus on shared decision-making
- Experience in primary care or geriatric care (preferred)
