Job Title: Population Health Clinical Navigator
Department: Greater Rochester Independent Practice Association (GRIPA)
Location: 100 Kings Highway South, Rochester, NY
Hours Per Week: 40 hours
Schedule: Full-time, Days
This position will be employed by the Greater Rochester Independent Practice Association, INC. (GRIPA). GRIPA was founded in 1996 as a unique collaboration between hospitals and physicians, for the purpose of simultaneously improving the quality and efficiency of health care. This position will not be employed by Rochester Regional Health (RRH).
SUMMARY:
The Population Health Clinical Navigator is part of the multidisciplinary Care Management team. Responsibilities include outreach to patients to educate them on appropriate utilization of health care services. Requires collaboration with GRIPA and RRH care managers, physicians, other entities (insurance companies) and all other health care professionals at various sites of service.
RESPONSIBILITIES:
Maintains a keen understanding of attribution, value-based contracts and end goals.
Continues to grow in knowledge of community resources.
Ensures timely and complete documentation following any patient and/or provider related contact per department documentation policy/procedure within electronic medical records to manage the population and promote care team communication.
Reports on successes as well as areas where there are opportunities for improvement.
Possesses and continues to build skills to enhance cold calls to patients.
Educates patients about appropriate utilization of health care services.
Educates patients about improving communication with the health care team.
Educates patients about select health screenings and conditions.
Empowers patients to solve problems, explore options of care and choose options that best fit their personal goals and desired outcomes.
Directs patients to participate in use of community resources.
Participates in employer contract specific initiatives, in collaboration with the care management team.
Reports on successes as well as areas where there are opportunities for improvement.
Engages in performance improvement activities to promote quality care.
Ensures timely and complete documentation into all information systems per department documentation policies.
Consistently presents a professional image through personal interactions, telephone and e-mail communications, and appearance.
Adheres to ethical, legal and practice standards of care management practice.
Self-monitors quality metrics to promote positive population health outcomes.
REQUIRED QUALIFICATIONS:
3-5 years of experience
Two-year AAS Degree and/or comparable experience required
Prior experience in a health care setting, and/or related business, physician office or hospital setting
PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
PAY RANGE: $19.75 - $26.00
The listed base pay range is a good faith representation of current potential base pay for successful applicants. It may be modified in the future. Pay is determined by factors including experience, clinical licensure date, relevant qualifications, specialty, internal equity, location, and contracts.
