The Organization


San Gabriel/Pomona Regional Center (SG/PRC) is a private, nonprofit agency contracted with the California Department of Developmental Services (DDS) to provide services and supports to individuals with intellectual and developmental disabilities. We proudly serve 30 cities across the Foothill, Pomona, and El Monte Health Districts within Los Angeles County. Our services span the full lifespan—from infants and toddlers, school-age children, and transition-age youth to adults and aging adults. The Position


This position reports to the Nurse Manager, and provides compassionate and dedicated nursing coordination, consultation, and support to Regional Center staff, service providers, clients, and families. Core responsibilities include assessments, evaluations, consultations, and delivery of high-quality care, ensuring individuals served receive appropriate medical care and that staff and families are well-informed and supported in managing health needs.

Essential Job Functions

  • Provide nursing coordination and consultation to Regional Center staff, service providers, clients, and families regarding health and medical needs within the nursing scope of practice.
  • Offer support and information related to medical terminologies, disease process, medical and surgical procedures, and other health-related topics to enhance understanding and care planning.
  • Conduct evaluation visits and develop comprehensive nursing assessments to determine levels of care, respite needs, significant changes in condition, and appropriate facility placement.
  • Support, monitor, and prepare monthly reports for the ARFPSHN homes to ensure compliance and quality of care.
  • Coordinate with SG/PRC Medical Consultant and Client Consultation Committee to facilitate effective communication and decision-making regarding client health and medical services.
  • Create educational presentations for staff, parents, providers, and community groups on health-related issues and disease prevention.
  • Coordinate with Quality Assurance Unit and Placement Committee to determine if appropriate medical care was provided by the facilities.
  • Conduct a nursing death review to support M&M Committee in determining appropriate medical care provided by facilities.
  • Coordinate with local health care providers and hospitals regarding client health needs.
  • Act as an Executive Director Designee providing consents for medical and dental procedures for clients.
  • Performs other duties as assigned in support of departmental and organizational goals.
Requirements:

Employment Standards

  • Bachelor's degree in nursing (BSN) required.
  • Valid Registered Nurse (R.N.) license required.
  • Certificate in Public Health Nursing required.
  • Four years of progressively responsible experience in public health nursing or a combination of training and experience that provides the desired knowledge.
  • Master’s degree in a related field can be substituted for one year of the required experience.

Knowledge and Abilities

  • Proven knowledge of the characteristics and healthcare needs of high-risk infants and individuals with developmental disabilities.
  • Strong understanding of general nursing care and scope of practice.
  • Experience with methods and techniques used in acute care nursing facilities.
  • Knowledge of quality management principles and practices.
  • Excellent training, communication, and presentation skills.
  • Ability to plan, organize, and manage multiple responsibilities effectively.
  • Flexibility to adapt to evolving priorities and clinical demands.
  • Compassionate, patient-focused approach with strong interpersonal skills.
  • Ability to adhere to the highest standards of quality in an efficient manner.

Other Essential Requirements

  • This position is a community-based position that requires meetings in the community, or in our office location.
  • Frequent telephone, virtual, and out-of-office meetings are required.
  • A valid driver’s license, reliable transportation, and minimum liability insurance coverage is required.

Physical Demands & Work Environment

  • Works 80% of the time in an office environment and 20% in the community at meetings or home visits.
  • Ability to drive to and from meetings or home visits and walk from car to and from appointments.
  • Occasional walking and standing to attend and participate in office meetings and trainings.
  • Ability to sit for extended periods and operate a computer for report writing
  • Occasional lifting, pushing, or pulling of items up to 25 pounds.
  • Adequate visual acuity to read documents, use a computer monitor for prolonged periods, and review detailed information.
  • 60% of the work time occurs in a typical office setting with standard lighting, temperature, and moderate noise levels.
  • Employees are expected to follow safe and ergonomic work practices.
