Community Resource Specialist in Bolivar, Missouri at Citizens Memorial

Citizens Memorial Hospital (CMH) is recruiting a Community Resource Specialist to work with our amazing team in Bolivar, Missouri — someone who leads with skill, compassion, and a drive to positively impact every patient and family we serve.

Position Details

• Status: Full-time
• Work Shift: Days
• Hours: Monday-Friday, 8am-5pm

What Your Day Looks Like: In this role, you will support patients navigating the healthcare system, removing barriers to care, and referring to community resources to address social determinants to health for patient safety.

As a Community Resource Specialist, you will be:

• Providing direct support to patients and families by addressing socio-economic factors impacting health, both in-person and by phone.
• Receiving referrals for specialty social service support for clinic patients and providing timely follow-up.
• Performing social needs screening and advising of applicable resources and solutions available.
• Documenting patient-centered plans of care.
• Collaborating and coordinating care with an interdisciplinary care team and advocating for patient safety in complex and challenging situations.

Qualifications

• Education: High school diploma
• Skills: Written and oral communication skills, time management, detail-oriented
• Licensure/Certification: Willingness to complete CHW certification within 6mo of hire; Preferred: Current CHW Certification
• Physical Requirements: Prolonged periods of sitting to perform computer tasks
