Job Description

About Wellness Equity Alliance

Wellness Equity Alliance is a national multidisciplinary health organization that designs and delivers integrated, community-based care for populations most impacted by health inequities. We do this through mobile and field-based models, providing medical care, behavioral health services, substance use treatment, harm reduction, and care coordination in nontraditional settings such as encampments, schools, reentry sites, and rural communities, as well as with sovereign tribal nations. Grounded in trauma-informed, culturally responsive, and data-driven practices, WEA combines clinical expertise, lived experience, and advanced population health analytics to reduce barriers to care, improve continuity, and strengthen local systems. We have partnered with more than 60 public agencies, managed care plans, and community-based organizations across the U.S. to implement scalable, sustainable programs that are advancing health equity and improving outcomes for historically marginalized populations.

We are known as Renegades, Rebels, Disruptors, and Dreamers. If that sounds like you, we want you on our team.

Position Summary

WEA is hiring an HIV Street Medicine Program Manager to lead the initiative’s field operations and network development. In field operations, the Program Manager makes sure new teams launch successfully and keeps workflows streamlined and efficient. In network development, the Program Manager builds the regional and national collaboratives that sustain our field teams and allow the model to grow. This role suits someone who wants to stay with a program for the long run, building relationships with local, regional, and national stakeholders and refining operations as the initiative scales.

Location:

In-person in Los Angeles, San Bernardino, Indio, or San Diego, CA, with frequent travel to field sites across California and other states as the initiative expands.

Key Responsibilities

Launching Field Teams

  • Lead new team launches from assessment through steady state, with guidance from the Director of Street Medicine.
  • Build and maintain a standard launch playbook so each new team starts faster and with fewer gaps than the last.
  • Conduct local stakeholder outreach and establish referral pathways with shelters, harm reduction programs, housing providers, behavioral health agencies, emergency departments, police departments, public health departments, and local community-based organizations (CBOs).
  • Support the Director of Street Medicine with team lead check-ins to resolve operational issues, refine workflows, and support staff retention.

Building Regional and National Collaboratives

  • Conduct outreach and follow-up with city, county, and state health departments to convene regional and statewide HIV street medicine collaboratives.
  • Engage FQHCs, HIV/AIDS service organizations, academic medical centers, tribal health programs, Ryan White recipients, and community-based organizations.
  • Shape collaborative governance, charters, shared goals, and data-sharing agreements alongside partners.
  • Facilitate learning communities that spread best practices, share protocols, and train new street medicine teams.
  • Identify and assess new markets for expansion, including urban, rural, frontier, and tribal communities.
  • Represent WEA at national conferences, policy convenings, and funder briefings as a credible voice on street-based HIV care.

Program Operations, Data, and Funding

  • Manage the program budget and annual work plan in partnership with the Director of Street Medicine, Chief Development Officer, and Chief Financial Officer.
  • Monitor outcome measures across HIV Street Medicine programs, including tests performed, new diagnoses, PrEP starts, linkage to care, retention, and viral suppression.
  • Make sure all teams report consistent monthly data on core HIV care continuum measures.
  • Lead quality assurance and quality improvement (QA/QI) efforts.
  • Maintain compliance with HRSA, Ryan White, CDC, state, and county requirements, and support audits and site visits.
  • Prepare grant reports and contribute program content to proposals with the Development team, making sure all reports are submitted on time with no compliance findings.
  • Cultivate funder relationships and help identify new public and private funding, under the guidance of the Chief Development Officer and with support from the Development team.
  • Gather stories, images, data, and spokesperson support for WEA communications while protecting patient dignity and privacy.
  • Keep the CEO, CMO, CDO, and Director of Street Medicine informed on the status of all launches, collaboratives, and outcomes.

Minimum Qualifications

  • Bachelor’s degree in public health, social work, nursing, health administration, or a related field.
  • Five (5) or more years of experience managing community health programs.
  • Proven record of launching new programs or sites and leading them to stable operation.
  • Valid driver’s license and ability to travel frequently, including overnight and out-of-state travel.

Essential Skills

  • Working knowledge of the HIV care continuum, PrEP and PEP, and Ryan White or HRSA program requirements.
  • Experience building coalitions or multi-partner collaboratives with shared goals.
  • Strong skills in budgeting, data-driven management, and grant reporting.
  • Strong organizational skills, with the ability to manage multiple programs and partners at once.
  • Exceptional written and verbal communication skills to engage partners, funders, and communities and to represent WEA.
  • Strong commitment to quality assurance and quality improvement.

Preferred Qualifications

  • Master of Public Health (MPH).
  • Experience working with government agencies.
  • Knowledge of services for people experiencing unsheltered homelessness.
  • Knowledge of harm reduction and trauma-informed care principles.
  • Comfort working in outdoor street-based settings, encampments, shelters, and rural sites, as well as in an office using virtual meeting and planning tools.

Work Environment

This position combines office-based work with regular field work in outdoor street-based settings, encampments, shelters, and rural communities, with frequent regional and out-of-state travel.

Employment Conditions

Employment is contingent on successful completion of a background check following a conditional offer of employment and on meeting WEA’s Occupational Health Clearance requirements. Qualified applicants with arrest or conviction records will be considered for employment in accordance with the California Fair Chance Act and applicable local fair chance ordinances.
