Job Description
The Center Incident Manager is responsible for ensuring the adherence to policy, procedures, and protocols as it relates to safety and incident management. The Center Incident Manager oversees the review, analysis, management, and finalization of incident reports, grievances, and corrective action processes while promoting continuous quality improvement. The Center Incident Manager will be responsible for assisting the PA Residential Program and Education departments in the continued compliance of all local, state and federal laws and regulations. The Center Incident Manager role will focus on the development and implementation of systems for the analysis of critical incident reporting data, corrective action plans, and outcome reporting.
Job Duties/ Essential Functions
- Review, analyze, manage, and finalize all PA Residential incident reports by evaluating policy and protocol consistencies, compiling data, analyzing data for recommendations toward improved safety and regulatory compliance. Ensuring timely finalization of the entire incident packet including but not limited to, Incident Report, Restraint Progress Report, Staff and Client Debriefings, Incident Case Management Notes, HCSIS reports, required referral source reportable forms and anything else required per incident.
- Oversee the Client Grievance process, all required follow up documentation and to identify trends, risks, and opportunities for continuous improvement. Communicate and collaborate with center leaderships on minimizing occurrences for potential client rights infractions and increase quality care.
- Analyze Quality Assurance and related data, in order to produce graphic, tabular and narrative reports for residential needs.
- Assist in the preparation of any upcoming on site/remote audit by completing documentation review, incident review and policy adherence checks and physical plant walkthroughs.
- Develop, implement and track corrective action plans to ensure compliance with state, local and federal requirements. Ensure timely completion and submission of all required documentation to the respective licensing oversight body.
- Assist Residential Leadership in incident review, including collection of documentation and internal review of incidents which may include video reviews and/or gathering witness statement when needed.
- Prepare outcome reporting and presentations on incident trends, findings and recommendations for management and stakeholders.
- Ensure adherence to organizational policies, procedures and safety standards across residential programs.
- Provide administrative oversight of required safety and contraband checks from each house/center. Identify deficiencies with prohibited items, client inventory procedures and/or physical plant hazards, work collaboratively with center leaderships and respective support departments to reduce opportunities for incidents.
- Identify deficiencies and opportunities for formal and refresher trainings; ensure implementation and completion of each training is completed and monitor trends after implementation for data outcomes.
- Assist in promoting safety and consistency in programming throughout assigned services.
