Job Description

SUMMARY DESCRIPTION

The Tobacco Specialist position will provide tobacco cessation counseling, treatment, and recovery support to patients attempting to overcome their nicotine addiction as stated in the HCF Tobacco Grant Tobacco Treatment Specialist will provide direct patient care services to tobacco users of the Hamakua-Kohala Health clinics and provide these services to the Hamakua and North Hawaii communities through outreach.

REPRESENTATIVE DUTIES

The following duties are typical for the TOBACCO CESSATION TREATMENT SPECIALIST-2 Incumbents may not perform all of the listed duties and/or may be required to perform additional or different duties from those set forth below to address business needs and changing business practices.

• Will work closely with other tobacco cessation treatment specialist.

• Provide Tobacco Cessation services which include: brief intervention, Intensive intervention development of tobacco quit plans, relapse counseling, tobacco quit maintenance, and recovery via face-to-face and/or phone support.

• All counseling techniques and practices being evidence based.

• Provide evidence based medication counseling with proper usage to patients wanting to quit using medication in cooperation with their PCP.

• Provide cessation services for users of electronic smoking devices.

• Provide evidence based appropriate Nicotine Replacement Therapy (NRT) to be provided in cooperation with the patient’s PCP.

• Maintain appropriate patient records for tracking patient contacts, providing evaluation information to HCF and their designated entities, Hamakua-Kohala Health required patient documentation through the EHR, and other needed records

• Attend and participate in meetings and events that promote Hamakua Health Center‘s tobacco cessation program..

• Keep current with tobacco cessation techniques, counseling, and medications through evidence based education and training.

• Maintain working relationships with Behavioral Health, Diabetes Care, PCPs, and other HHC departments to provide tobacco cessation for their patients.

• Provide appropriate reporting of program successes and failures on a timely basis to supervising personnel.

• Create and/or acquire appropriate educational materials to provide aid to patients in their cessation of tobacco.

• Maintain professional standing in tobacco cessation through ongoing educational training.

EDUCATION, EXPERIENCE & QUALIFICATION GUIDELINES

Any combination of education and experience that would likely provide the required knowledge and abilities is qualifying.

• Tobacco Treatment Specialist Certification which must be obtained within the first 6 months of employment if not already certified. (Department of Health and HCF will provide training through the grant)

• Certification in Tobacco Brief Intervention to be obtained as soon as possible upon employment if not already certified.

• Experience in doing reports, using an EHR (electronic health records), and outreach is preferred.

• Data review and Analysis using data queries from EHR

PERFORMANCE REQUIREMENTS

• Courteous, honest and professional at all times.

• Able to communicate and relate well with physicians, clinical support staff and other HKH employees.

• Able to provide innovative input into the development of the clinic and its processes.

• Maintains appropriate licensure and certification including meeting and/or exceeding continuing education requirements.

• Efficient, organized and accurate.

SUPERVISORY RESPONSIBILITY

This position has no supervisory responsibility
