Supportive Concepts for Families is seeking compassionate Licensed Practical Nurses (LPNs) to serve as Community Health Nurses, providing ongoing clinical support and care coordination for adults with intellectual and developmental disabilities living in community homes throughout Pennsylvania.

This is a field-based nursing role where you'll work independently while collaborating with Community Health RNs, physicians, specialists, Direct Support Professionals, and interdisciplinary teams to ensure exceptional care for the individuals we support.

What You'll Do
  • Manage a caseload of individuals with routine and stable medical needs.

  • Provide ongoing nursing oversight and wellness monitoring.

  • Administer medications and treatments in accordance with physician orders and your scope of practice.

  • Coordinate medical appointments and communicate with physicians, pharmacies, specialists, and families.

  • Monitor chronic health conditions and promptly report changes in condition to the Community Health RN and healthcare providers.

  • Review physician orders, medication changes, laboratory results, and clinical documentation.

  • Educate and support Direct Support Professionals on medication administration, health monitoring, infection prevention, and medical protocols.

  • Participate in interdisciplinary team (IDT) meetings.

  • Maintain accurate documentation and ensure compliance with state, federal, and organizational standards.

  • Collaborate closely with Community Health RNs to support individuals with higher medical complexity.

Qualifications
  • Active Pennsylvania Licensed Practical Nurse (LPN) license in good standing.

  • Valid driver's license and reliable transportation.

  • Strong clinical assessment, communication, and organizational skills.

  • Ability to work independently while collaborating with an interdisciplinary healthcare team.

  • Experience in IDD, developmental disabilities, long-term care, home health, behavioral health, or community nursing is preferred but not required.
