Shift: Mon-Fri, 7:30am-4:00pm

Summary of Job Duties: Our Nurse Practitioner (NP) is responsible for periodic and episodic assessment and management of individual participants’ medical conditions, and for oversight of their use of medical specialists and inpatient care. The NP is part of an Interdisciplinary Team with the knowledge and skills necessary to assess, plan care for, and provide service to frail elderly participants.

Required Education: Completion of an accredited, Board-approved, Master’s, Post Master’s or Doctoral Nurse Practitioner program.

Required Licensure:

Pennsylvania RN licenseCRNP certification in Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) or Family Nurse Practitioner (FNP), Pennsylvania Prescriptive Authority Approval DEA license. Valid PA Driver’s license.All licenses must be current, active and unrestricted.

Required Experience: A minimum of 1 year providing services to a frail or elderly and/or mentally and physically handicapped population preferably in a health care setting or an equivalent combination of education and experience. A minimum of 3 years of NP experience is highly preferred.
