Initiative and excellent interpersonal and organizational skills required. Marketing and community relations will round out this great opportunity. Bachelor of Social Work required, Master of Social Work and long-term care experience preferred.

• Admission assessments and agreements
• Care Plan Coordination
• Process and discharge planning
• Medicaid applications
• Grievances
• Other duties as assigned
