Core - APC Remote Primary Care Inbox in Exeter, New Hampshire at Exeter Hospital

Support primary care teams remotely by completing indirect patient care tasks. Remote inbox providers collaborate with on-site providers and staff to co-manage the medical needs of patients.

Requirements:

• Master’s Degree
• 3-5 years’ clinical experience in primary care, coverage for all ages is required.
• Board-certified APRN, PA-C
• DEA
• NH License
• New England Residency preferred

Major Responsibilities:

1. Respond to patient calls and portal messages requiring provider input.
2. Review and refill medication refill requests.
3. Coordinate patient care with triage and care coordination nurses.
4. Communicate lab and diagnostic test results to patients.
5. Complete peer-to-peer review with insurance company for authorization of testing or treatment when needed.
6. Attend regularly scheduled virtual meetings with on-site providers.
7. Other duties as assigned.
