Community Home Care Coordinator in Horseheads, New York at Bethany Village

Position Overview:

Responsible for the assisting with overall direction of the Licensed Home Care Nursing Department; manage and supervise nursing functions, assess quality and quantity of performance of staff members reporting to him/her, and oversee client care in accordance with the resident, physician's orders, and family needs.

Essential Duties & Responsibilities:

• Coach & lead nursing staff, and train RN’s and LPN’s to provide appropriate team leadership and to practice all procedures as established.
• Interview, recommend potential hires, orient & supervise nursing staff.
• Direct clinical supervision of medication management, blood glucose monitoring, insulin administration; wound care, and all other clinical services.
• Responsible for facilitating data collection, analyses, and disseminating information related to Quality Assurance and Professional Standards Program.
• Ensures proper documentation is completed by Personal Care Aide 2 and Home Health Aide staff.
• Work in conjunction with the Bethany At Home Coordinator to provide ongoing orientation education, and in-service to staff on a regular basis.
• Monitors RN and LPN services in the Community and at Appleridge Senior Living.
• Supervise/evaluate staff for conformity to company policies and procedures, laws and regulations, and professional standards; ensure that nursing assignments / reports are completed & communicated to incoming shifts appropriately-assist with hands-on duties as necessary.
• Communication with referring health care team, including condition, goals, changes in care.
• Perform physical assessments of residents.
• Assist with physician rounds.
• Will be responsible for the Community Home Care Services operation at Appleridge and in the community.
• Responsible for taking new referrals, evaluating potential clients, negotiating rates for billing, admissions, care planning, physicians’ orders, nursing assessments and coordination of aide and nursing services that is appropriate for each individual client.

Basic Duties & Responsibilities:

• Ensures nursing equipment/emergency equipment is in proper working condition; perform appropriate equipment audits.
• Assists with staff immunizations/ppd/ Health Assessments.
• Serves as clinical resource for all home care staff.
• Promote & participates in safety committee / programs.
• Participate in weekly team meetings and management meetings as required.
• Infection Control educator
• Emergency preparedness and knowledge of all procedures and policies.
• Liaison between all outside agencies in respect to nursing; work with campus and community departmental managers to ensure that all aspects of the clients’ needs are met and carried out in accordance with mandated regulations.
• Ensures that care procedures are carried-out in compliance with all state, DOH, and OSHA regulations; random audits performed as needed; update policy and procedures as needed.
• Endorses and complies with the vision, objectives, procedures, and policies of Bethany Village.
• All other duties as assigned.

Education Required:

• RN license & CPR certified
• BS in Nursing/related field preferred

Experience Required:

• Previous geriatric/dementia/long-term care experience.
• 2-5 years supervisory/management experience preferred
