•Develop and implement workflows, protocols, and standard operating procedures for crisis screening, stabilization, disposition planning, and transitions to inpatient, residential, outpatient, or community-based services.
•Coordinate closely with medical staff, mobile crisis teams, hospitals, health plans, and community partners to facilitate rapid admissions, consultations, and transfers while minimizing unnecessary emergency department or inpatient utilization.
•Maintain real-time oversight of census, throughput, and barriers to discharge or placement; intervene operationally to resolve delays and ensure timely, clinically appropriate transitions.
•Monitor documentation, regulatory compliance, and quality metrics related to crisis stabilization services, including length of stay, engagement, and successful linkage to follow-up care.
•Lead, coach, and support crisis engagement and care coordination staff, providing ongoing training in crisis assessment, de-escalation, trauma-informed care, and disposition planning.
•Foster strong interdisciplinary collaboration and clear communication across shifts and departments to ensure continuity and safety of care.
•Use data and performance trends to drive quality improvement initiatives that enhance access, patient experience, operational efficiency, and clinical outcomes across the crisis continuum.
•Additional Duties as assigned.

•Knowledge of care coordination processes and best practices.
•Understanding of healthcare regulations and quality standards.
•Excellent communication and interpersonal skills.
•Ability to collaborate and work effectively with interdisciplinary teams.
•Analytical and problem-solving abilities.
•Strong organizational and time management skills.
•Proficiency in healthcare information systems and technology.
•Ability to adapt to changing healthcare environments and priorities.
