Fellowship Therapy at Home is redefining outpatient care by delivering high-quality therapy services directly to clients in the comfort of their homes. We bridge the gap between home health and clinic-based therapy—offering therapists flexibility and clients personalized care where it matters most.

We are seeking a compassionate and motivated Per Diem Occupational Therapist to join our growing team.

Responsibilities

  • Provide in-home, community-based OT services
  • Develop and implement personalized treatment plans
  • Help clients improve functional independence in real-world settings
  • Document care and progress in a timely, compliant manner
  • Collaborate with a multidisciplinary team
  • Educate clients and caregivers on safety, adaptive strategies, and wellness

Qualifications

  • Current New Jersey Occupational Therapist License (Required)
  • Minimum 1 year of experience in outpatient or home health (preferred)
  • Strong communication and time management skills
  • Ability to travel between client locations throughout the day
  • Self-motivated and comfortable working independently
