Fellowship Therapy at Home is redefining outpatient care by delivering high-quality therapy services directly to clients in the comfort of their homes. We bridge the gap between home health and clinic-based therapy—offering therapists flexibility and clients personalized care where it matters most.
We are seeking a compassionate and motivated Per Diem Occupational Therapist to join our growing team.

Responsibilities
- Provide in-home, community-based OT services
- Develop and implement personalized treatment plans
- Help clients improve functional independence in real-world settings
- Document care and progress in a timely, compliant manner
- Collaborate with a multidisciplinary team
- Educate clients and caregivers on safety, adaptive strategies, and wellness
Qualifications
- Current New Jersey Occupational Therapist License (Required)
- Minimum 1 year of experience in outpatient or home health (preferred)
- Strong communication and time management skills
- Ability to travel between client locations throughout the day
- Self-motivated and comfortable working independently
