Sylva Valley Health & Rehab is hiring an experienced RN MDS / Case Management Nurse to lead accurate assessments, strong clinical documentation, and coordinated care in our skilled nursing setting. This role is ideal for a nurse who is detail-oriented, deadline-driven, and confident managing MDS timelines while collaborating with the interdisciplinary team.

• Coordinate and complete MDS assessments accurately and on time (OBRA/PPS as applicable)
  
• Manage ARD scheduling, tracking, and compliance timelines
  
• Ensure documentation supports skilled services and reimbursement (PDPM/PPS focus)
  
• Participate in/lead IDT and care plan coordination
  
• Support case management needs: clinical updates, payer communication, continued stay reviews as needed
  
• Collaborate with therapy, nursing, providers, admissions, and leadership to ensure appropriate level of care and accurate documentation
  
• Maintain compliance with facility policies and applicable regulations
  

• Active Registered Nurse (RN) license
  
• MDS experience required in a skilled nursing facility
  
• PointClickCare experience required
  
• Strong knowledge of MDS processes, care planning, and skilled documentation
  
• Excellent organization, communication, and time management skills
  

• Experience with PDPM/Medicare skilled documentation and managed care reviews
  
• MDS certification
