JOB PURPOSE/SUMMARY

Summary of position/role of team:

  • This position requires the Transition Campus RN to serve as a clinical member on the multi-disciplinary treatment team providing support, rehabilitation, and emotional support for their designated client population.
  • The Transition Campus RN will provide direct psychiatric and medical care to admitted clients in the Residential setting including varying levels of crisis, SPMI populations and co-occurring disorders.
  • Responsible for ensuring that each client's mental health and medical health needs are met via the completion of medical assessments, administration of medications, collection of vital signs, etc.
  • The Transition Campus RN will conduct thorough assessments and will assist with crisis interventions and medical emergencies.
  • The position requires adequate implementation of verbal and de-escalation techniques.
  • The Transition Campus RN must be adept at multi-tasking.

TYPICAL WORKING CONDITIONS/ENVIRONMENT

  • The Transition Campus RN will provide psychiatric services in one or more designated crisis/residential programs, mainly the Blount County Transition Campus Detox/Residential units despite any weather conditions that may arise.
  • The Transition Campus RN will not provide services out in the community.

JOB DUTIES/RESPONSIBILITIES

1. Works in a multi-disciplinary team approach to meet the clinical, medical, and social needs of patient who present in a psychiatric emergency.

  • Completes all daily task and general duties as assigned pending client and program needs
  • Attends daily treatment team meetings
  • Assist with providing coverage when needed
  • Follows all established protocols and guidelines for admission prior to admitting the client to the unit.
  • Performs initial admission and nursing assessment including all required paperwork.
  • Updates diagnoses, medications, and allergies in the EMR
  • Obtains UDS, BAL, and pregnancy test as indicated upon admission
  • Makes the determination of the appropriate admission protocols needed for each client
  • Obtains admission orders from the provider on duty or physician on call.
  • Assist with administering medications and documentation must reflect this in the EMAR
  • Collaborate effectively and clearly with other team members around patient care issues and documents this in the patient medical record when appropriate
  • Attend treatment team meetings and provide input on patient care and discharge planning.
  • Provides education and discharge instructions with client and/or family.
  • Collaborate with outside healthcare professionals surrounding their discharge care or other referral sources including higher levels of care.
  • Counts controlled substances and documents at every shift change and notify your supervisor immediately if there are any discrepancies.
  • Participates and provides thorough shift reports.

2. Completes all documentation in compliance with CARF and HRMC standards.

  • Completes and signs all documentation for each client prior to end of shift
  • Respond to all flags, emails, and voicemails from other clinical staff by end of shift. The RN is to return calls within 15 minutes
  • Document administered medications in the EMAR
  • Document clinical services provided in the medical record on same day as service provided in a clear and concise manner.

3. Completion of required HRMC training to maintain status as credentialed provider.

  • Maintain an active RN license in the state of Tennessee in accordance with the state guidelines
  • Complete any Center Required Self-Directed Learning or Training Modules in the required time frames.
  • Complete First Aid, CPR, and HWC trainings within the required time frames.

COMPENSATION:

  • Starting salary for this position is approximately $30.87 /hr based on relevant experience and education.

Schedule:

  • The Blount Transition Campus operates 24/7, therefore the RN must be flexible with scheduling as there is the possibility of overnight shifts and holiday work.
  • The schedule of this position is normally Monday- Friday, 40 hours per week, with varying start and end times as needed.
  • Days and shifts worked can vary based on client and/or programmatic needs and requirements of program.
  • This position is residential, which requires working holidays and the position is expected to participate in an on-call rotation.
  • This on-call rotation could involve weekend and after-hours work.
  • Regular attendance is an essential job function.
  • Must be punctual and maintain a good attendance report.
  • Must provide a six-hour notice if unable to work their scheduled shift (unless a documented emergency arises).
  • The Transition Campus RN must stay awake and alert on all shifts.
  • The Transition Campus RN is expected to fulfill the agreed upon hours when hired.
  • Unless otherwise approved, the Transition Campus RN is expected to be present on the campus for the full shift.
  • All other scheduling arrangements are approved on a case-by-case basis.

Travel:

  • Travel is not required if the Transition Campus RN is providing medication services to their designated crisis program.
  • The Transition Campus RN may have to travel between clinics/programs at times, and may be required to travel for any supervision, trainings, or scheduled meetings.
  • There is no overnight travel.

Equipment/Technical Competency:

  • Basic computer skills required (EMR, Excel, Word, etc.). The CP-RN will need access to e-mail, Athena Health (EMR), I & U: Drives and Drs. First.
  • Telehealth services may be offered as part of the service delivery.

Equipment/Technology:

  • The Transition Campus RN will require a desktop to provide clinical services.
  • It is essential for the Transition Campus RN to have access to this device in order to access the EMR, Drs. First, and telehealth services.

QUALIFICATIONS - Blount Substance Use RN

Experience / Knowledge:

  • Prior clinical experience is a plus.
  • Experience in behavioral health and substance abuse preferred.
  • Should possess sound knowledge of addiction services, psychiatric medications, diagnoses, treatment modalities and interventions.
  • Must have the ability to present professionally, work within a team format, exercise sound clinical judgment under pressure, communicate effectively, possess good time management, organizational and clinical skills.
  • Knowledge of HIPAA, confidentiality and patient centered practice approach required.
  • Experience with interdisciplinary collaboration and care coordination preferred.

Education / License:

  • Must have graduated from an accredited school of nursing and possess either an Associates or Bachelor's degree in nursing.
  • Must be actively licensed as a Registered Nurse in the state of Tennessee.
  • American Nursing Association certification as a Psychiatric Mental Health Nurse preferred.
  • Must have Certificate of Fitness from the Tennessee Board of Nursing.

Physical/Emotional/Social - Skills/Abilities:

  • Staff must complete and maintain First-Aid and CPR trainings within the required time frames.
  • Transition Campus RNs must complete the full Handle With Care Trainings as indicated.
  • Some exposure to biological hazards and unpleasant odors.
  • Hearing of normal and soft tones.
  • Close eye work.
  • Lifting up to 50 lbs.
  • Pushing/pulling up to 150 lbs.
  • Frequent sitting, standing, walking, bending, stooping, and reaching.
  • This position requires training and certification in and the ability to adequately implement a variety of verbal de-escalation methods and personal/client safety techniques.
