About This Position

JOB SUMMARY

A versatile outpatient pharmacist with comprehensive expertise in all sub-fields of community-based pharmacy practice including but not limited to Long-Term Care, 340b, Specialty, & Medication repackaging. Demonstrates the ability to transition seamlessly between workflows, staffing models, and operational environments if needed. Strives for superior performance by consistently providing a product or service to leadership and staff that is recognized as ultimately contributing to the patient and family experience. Recognizes and demonstrates understanding of patient and family centered care.

PRIMARY JOB RESPONSIBILITIES

    Evaluates physician drug orders for appropriateness with regard to patient condition, organ function, disease state, and concomitant therapies to promote optimal patient outcomes. Includes, collection and interpretation of information regarding patients’ allergies, organ function, drug history, and other information pertinent to drug therapy. Processes physician drug orders including, but not limited to, order entry into dispensing computer system, appropriate product selection, compounding, labeling and repackaging with completion of appropriate records; delegating product preparation activities to pharmacy technicians when appropriate, within legal limits and health system policies and procedures. Pharmacists must be able to demonstrate the knowledge and skills necessary to provide care and monitor appropriate to the age of the patient populations served, including age related, renal, and hepatic drug metabolism Consults with various health care professionals and provides information and guidance concerning drugs and their appropriate use. Ensures drugs are reasonably available to patients when needed to avoid delays in therapy and promote positive patient outcomes. Complies with departmental and health system policies and procedures. Assumes duties of Director of Pharmacy in his/her absence to manage daily departmental activities. Assists in ensuring that drug items are appropriately billed. Assists in ensuring that pharmacy inventory is maintained at appropriate levels. Reviews and assumes responsibility for products prepared by pharmacy technicians and assigns tasks to pharmacy technicians as necessary to meet workload demands. Participates in and/or provides support to hospital committees as agreed upon by committee members and the Director of Pharmacy to help establish drug-related policies, criteria, or to monitor issues related to drugs. Assists the department and health system in maintaining compliance with accreditation standards. Demonstrates knowledge of and supports health system mission, vision, value statements, standards, policies and procedures, operating instructions, confidentiality statements, corporate compliance plan, customer service standards, and the code of ethical behavior.

ADDITIONAL RESPONSIBILITIES

    Advises and assists with implementing and maintaining mechanisms ensuring compliance with applicable HRSA rules and regulations, specifically regarding the 340b drug program. Assists with developing and implementing software and workflow improvements and adjustments to maintain and improve upon outpatient pharmacy operations. Assists with overseeing long term care program not limited to, but including: Assisting in ensuring drug storage locations are maintained appropriately (e.g., floor stock, automated dispensing systems, etc.). Overseeing filling of patient medpacks and development of relevant workflows. Working with long term care personnel to ensure accurate charting of patient medication orders and delivery of updated orders.
Complies with universal precautions and other safety procedures related to hazardous drugs, chemicals and biologicals. Supervises and assists with onsite onboarding and training of new pharmacy employees. Maintains proficient working knowledge of applicable pharmacy software Participates in drug education activities for patients and staff. Attends applicable departmental staff meetings. Fulfills any other additional duties not specifically enumerated, as directed by the Pharmacy Director.

JOB SPECIFICATIONS

EDUCATION

    Graduate of a College or School of Pharmacy accredited by the Accreditation Council for Pharmacy Education with a minimum of a Bachelor's Degree is required. Maintains licensure in good standing as a Registered Pharmacist in Michigan is required. Basic Life Support (BLS) certification is required. Advanced Cardiac Life Support (ACLS) certification is preferred.

EXPERIENCE

    Ability to work individually and as a team member is required. Demonstrates effective verbal communication skills. Possessing basic computer skills is required. Proven ability to direct and supervise day to day work of Technicians is required

ESSENTIAL PHYSICAL ABILITIES/MOTOR SKILLS

    Job tasks are performed in close physical proximity to other people Requires working indoors under environmentally controlled conditions. May include exposure to disease or infections, hazardous materials, sounds and noise levels that can be distracting or uncomfortable. Must be able to read and interpret medication orders; work under stress, interruptions, and deadlines; detect medication order discrepancies and must be able to correct them with approval of the physician; assign and designate work, to problem solve, answer questions, and evaluate work results in order to effectively supervise pharmacy staff; answer pharmacy activity questions to non-pharmacy personnel; and recognize hazardous material symbols. Requires frequently repetitive movement; using hands to handle, push, pull, control, or feel objects, tools or equipment; standing for extended periods of time; lifting up to 50 pounds; walking, climbing stairs/ladders. Able to travel independently throughout all Memorial Healthcare facilities. Small motor skills required for operating modern computer, office, and telephone equipment as utilized by Memorial Healthcare (MHC).

ESSENTIAL TECHNICAL ABILITIES

    Proficiency using modern office, computer and telephone equipment as used by Memorial Healthcare.

ESSENTIAL MENTAL ABILITIES

    Ability to adapt and maintain focus in fast paced, quickly changing or stressful situations. Ability to read and interpret a variety of documents including, but not limited to, policies, operating instructions, white papers, regulations, rules and laws.

ESSENTIAL SENSORY REQUIREMENTS

    Able to see for the purpose of reading information received in formats including, but not limited to, paper, computer, reports, bulletins, updates, manuals. Able to hear for work-related purposes.

INTERPERSONAL SKILLS

    Ability to interact with co-workers, hospital staff, administration, patients, physicians, the public and all internal and external customers in a professional and effective, courteous and tactful manner, at all times, physical, verbally and in all written and electronic communication. Required to remain calm when adversity is encountered. Open, honest, and tactful communication skills. Ability to work as a team member in all activities. Positive, cooperative and motivated attitude.
