Community Health Center of the North Country is seeking a Full-Time, highly skilled and compassionate Family Nurse Practitioner to join our health center team. As a Family Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to individuals of all ages. You will work collaboratively with our healthcare team to diagnose, treat, and prevent a wide range of medical conditions. PRIMARY RESPONSIBILITIES

- Provide routine and acute care to patients, including ordering and interpreting laboratory tests and imaging studies, conducting physical exams, and prescribing medications.
- Conduct health coaching sessions to educate patients on healthy habits, disease prevention, and wellness strategies.
- Collaborate with our physicians to provide comprehensive care to patients with complex medical needs.
- Utilize electronic health records (eCW) to document patient information and communicate with other healthcare providers.
- Participate in case management activities to ensure that patients receive the most effective and efficient care possible.
- Provide patient education and counseling to promote healthy lifestyles and disease prevention.
- Develop and implement treatment plans in collaboration with our physicians.
- Stay current with the latest medical research and guidelines, and adapt practice to meet the changing needs of patients and the healthcare community.

Requirements:

- Active, unrestricted NYS Family Nurse Practitioner license

- Two or more years of experience working as a Family Nurse Practitioner

- Experience working in an outpatient setting

- Strong knowledge of internal medicine principles and practices

- Ability to work effectively within electronic health record systems (eCW)

- Experience with case management and health coaching

- Strong communication and interpersonal skills

- Ability to work collaboratively as part of a multidisciplinary healthcare team

- No evenings, weekends, or call coverage required
